Defects in Westinghouse Circuit Breakers

NRC Bulletin 88-01 was issued on February 5,1988 informing licensees of safety concerns with Westinghouse Type OS circuit breakers. Licensees were requested to expeditiously inspect all Westinghouse DS-416 reactor trip and bypass breakers within thirty days of receipt of the bulletin and submit a letter of confirmation of completion of the inspections within sixty days.

The inspections were to be performed using the criteria given in NRC Bulletin 88-01.

Alabama Power Company has performed all required short and long-term inspections on the Joseph M. Farley Nuclear Plant, Units 1 and 2, reactor trip and bypass breakers. The inspections were completed on March 8,1988 and the results are attached.

Westinghouse DS-416 Breakers Inspected: Four Reactor Trip Breakers (two per unit)

Four Bypass Breakers (two per unit)

Results of Inspection:

Unit 1 a) Both reactor trip breakers passed criterion 6.1.1 (short-term inspection) and criterion 7.0 (long-term inspection),

b) Both bypass breakers failed criterion 6.1.1.

Cri terion 6.1.2 was applied and the breakers passed this criterion.

Both breakers passed criterion 7.0 (long-term inspection).

Corrective Action: The number of cycles on the Unit 1 bypass breakers will be monitored in accordance with criterion 6.2.2.

The pole shaf ts will be replaced when an adequate supply of pole shaf ts are obtained.

Unit 2 a) Both reactor trip breakers failed criteria 6.1.1 and 6.1.2.

Reactor trip breaker "B" f ailed criterion 7.1.

New pole shaf ts were installed in both breakers before the breakers were returned to service. The breakers were reinspected and passed criterion 6.1.1 (short-term inspections) and criterion 7.0 (long-term inspections),

b) Both bypass breakers f ailed criterion 6.1.1.

Bypass breaker "B" f ailed criterion 6.1.2.

A new pole shaf t was installed on Unit 2 bypass breaker "B" before returning the breaker to service. Bypass breaker "B" passed criteria 6.1.1 and 7.0 with the new shaf t installed. Bypass breaker "A" was subjected to criterion 6.1.2 af ter failure of criterion 6.1.1.

Bypass breaker "A" passed criteria 6.1.2 and 7.0.

Corrective Action:

The number of cycles on Unit 2 bypass breaker "A" will bc nonitored in accordance with criterion 6.2.2.

The pole shaf t will be replaced when an adequate supply of pole shaf ts are obtained.
