On October 20,'1987, members of the NRC-(D. Hood, D. Sellers and V. Hodge)-

called J. McVicker and S. Cowne of Baltimore Gas and Electric Company (BG&E) to discuss the Calvert. Cliffs Unit-1480 volt circuit breaker with a broken weldment connecting the' center pole lever to the pole shaft. Results of BG&E's examination are described in a September 18, 1986 memorandum (same as-i enclosure 2 of-0. Hood memorandum, " Notice of Meeting with Westinghouse Re-

'garding Class. IE Switchgear Models: DS-416, DSL-416, DS-420,.DS-206 and DSL 206," dated September 21, 1987, Docket Nos. 50-369 and 50-370).

BG&E responded to the following NRC questions:

Westinghouse Model DS-206 used in Class IE service for.the Unit 1 Control Room Habitability (Air Conditioning) System Q.

How detected?

During routine maintenance Q.

How did breaker last perform?

A.

Broken weld did not affect breaker performance

How many DS breakers are used at the facility?

A.

About 400 DS-206 breakers for both units.

Also use some 05-416 breakers.

Believe all of these to be Class IE.

Q. -Any other similar weld failures?

A.

None Q.

What is used for reactor trip breakers?

A.

GE AKs Q.

Why was a pole shaft removed from another breaker and sent along with the failed pole shaft to Materials Engineering and Analysis Unit for evaluation?

A.

It had bad appearing welds but was evaluated to be suitable for service.

Number of operating cycles at time of failure?

A.

Not known.

Breaker had operated for more than 2 years.

Failure probably happened in the first few cycles because it was an aged weld.

Fatigue was evidenced only in part of the weld associated with lack of fusion.

Any other reports on failed weld?

A.

Non conformance report 90386 and Maintenance Request 090286. Also Westinghouse reply by letter from E. Goslin and G. Steinel to J. McVicker dated November 17, 1986.
