SUBJECT:
WOLF CREEK NGS - COUNTERFEIT SAFETY-RELATED PIPE HANGER Enclosed is the Office of Investigations' final report of investigation on this subject. Our investigation substantiated an alleger's assertions that*a portion of a safety-related pipe hanger was fabricated from scrap material after the original bottom piece had been removed after having been found to have been improperly welded. The investigation further established that heat and hanger numbers were added to the nonquality piece in order to correspond with appropriate quality assurance documentation. The latter had the effect of deceiving the licensees' QC inspectors who relied on these numbers as well as the apparent quality of the welds in signing off on the counterfeit pipe hanger. Although the identity of the persons creating the counterfeit were easily ascertained (the alleger being one of them), we were not able to corroborate the alleger's claims that he was directed to counterfeit the hanger by his foreman. The investigation did not uncover knowledge of the oogus hanger by any licensee managers or supervisors. Essentially, the allegations of supervisory involvement is a stalemate, with the alleger's accusation balanced by the foreman's denial. 01 notes that the foreman had both the opportunity and motive (productivity being one neasure of his job performance) whereas a craft worker would not normally be concerned with productivity, per se. Further, the circumstances described by the alleger lend credibility to his story, and, on balance, the investigators v.ere unconvinced by the foreman's denials. Nonetheless, 01 dets not feel that the foregoing is sufficient to establish by preper.dcrar,ce of cvidence - supervisory involvement in this matter. It is also our view that additional investigative effort is not warranted given the fact that this appears t'o be an isolated incident. The nature of the case does not justify the use of a polygraph, and edciticntl interviews are unlikely to provide substantive information. Thus, cbsent cny new - information being brought to our attention, we consider this ir,estigation. closed.
