Architectural Specification A-125 and A-130 allows use of CZ-ll, manuf actured by carboline Corporation for touch-up of unprimed carbon steel. In order to enhance raceway support turnover, CZ-11 should be approved for touch-up of damaged galvanized surfaces. Recommended Action, Irelude Sketch, Etc. Recommend revising E-OR 8900, Paragraph 1.-7 to include approval of CZ-ll for touch-up of damaged galvanized surf aces of cable tray, exposed conduit , and support materials. This would allow one-step tou'ch-up of structrual steel and electrical supports and eliminate unneccessary work for craft.I

The allegations investigated and the summary of our findings are discussed in the enclosed investigation report." SUMMARY

An investigation was initiated at the Wolf Creek Nuclear Generating Station New Strawn, Kansas, following the receipt of allegation Gas and Electric Company. provided information of a violation to the NRC.The alleger stated he was termin He further alleged that his supervisor's standing order to him and his crew members was not to talk to NRC employees. altered after he was terminated.The alleger stated his personnel folder had been - and construction were interviewed and stated that the former employe terminated during a general reduction-of-force. Two personnel officers advised that the employees terminated were selected based on their record of attitudei workmanship, and absenteeism. Both personnel officers stated the alleger's personnel file documented he had received a written reprimand from his supervisor one week prior to the January 1982 reduction-of-force (ROF). A review of the allegers personnel file showed the alleger received Icw marks on attitude, quality of work, workmanship, dependability, and judgement on his

     " termination appraisal" which is executed after an employee is terminated.In addition,time wasting          the and alleger's  file disclosed that he was given a reprimand for loitering. Further review of the alleger's personnel file shcwed that the alleger's foreman listed him as eligible to be rehired; however, for rehire" due    the superintendent      changed the evaluation form to read "not eligible to alleger's low marks.

Three former co-workers of the alleger stated the alleger had developed a bad attitude and his work had

This refers to the investigation conducted by Messrs. Griffin, Driskill and Herr of the NRC Office of Investigations, Region IV Field Office during the period November 30,1982-January 14,1983, of activities authorized by HRC Construction Pemit CPPR 147 for the Wolf Creek Generating Station. This investigation was conducted to detemine .if untraceabie, nonquality material was substituted in a safety-related pipe hanger at the direction of the contractor's supervisory personnel. The investigation consisted of selective examination of procedures and representative records, interviews with personnel, and observations by the investigators. During this investigation, it was found that certain of your activities were in violation of NRC requirements. Consequently, you are required to .. respond to this violation, in writing, in accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2 Title 10. Code of Federal Regulations. Your response should be based on the specifics contained in the Notice of Violation enclosed with this letter. In addition, your response should address the following items specifically:

SUIEARY An investigation was initiated at the Wolf Creek Generating Station, Kansas Gas and Electric Company (KG&E), Burlington, Kansas, following the receipt of an allegation from a Daniel International Corporation (DI) employee concerning the substitution of scrap material by,a DI foreman for the original safety-related material in a pipe hanger. Another witness reporte'd that a DI general foreman ~ was made aware of a " bad hanger," but failed to take corrective action. Twelve past 'and present DI employees, believed to have knowledge of the inci-dent, were interviewed. A DI employee admitted removing the original bottom horizontal piece of a box hanger on March 16-17, 1981, replacing it with scrap material, and stamping heat and location numbers on the counterfeit piece utilizing tools in the field. This individual emphasized that he was only following the orders of the foreman. A former employee stated he assisted in replacing the piece with the understanding that the foreman had ordered this ' activity. Documents tracing the origination, fabrication, installation, and subsequent inspection of the hanger in question were requdted and received. A review of the traveler package for the hanger in question documented repairs to the hanger, but did not document the discarding of a portion of the original hanger or the installation of a counterfeit piece made from scrap material. The hanger in. question was removed by DI employees at the request of the NRC, Office of Investigations Field Office, Region IV. Subsequently, at the requesE of the NRC, the acting foreman of the DI pipe hanger fabrication shop examined the suspect piece and stated the numbers stamped on the piece contained various irregularities and were not stamped by personnel in the pipe fabri-cation shop. , One DI QC inspector who had initialed the traveler on March 17, , $ 1981, examined the stamped numbers on the alleged counterfeit piece and stated the numbers looked somewhat suspicious, and commented, "I missed it." Review of KG&E documents disclosed that this particular hanger was identified on a list of hangers that were reinspected during the summer of 1981. This i reinspection project occurred after another counterfeit hanger, identified in April 1981, was brought to KG&E's attention. The foreman and general foreman who were in charge of the crew that installed the suspect hanger were inter-viewed, and they denied any knowledge of the fabrication or installation of a , counterfeit hanger.
