1.
During a reinspection of installed concrete anchors at the Washington Nuclear Project No. 2 on 6/27/79, the licensee discovered that six anchor bolts had been cut short and tack welded to the support plates, giving the false appearance of proper installation.
The licensee has issued a stop-work order to the contractor involved, secured all related records, j,,,
and initiated a full investigation.

On June 28, 1979, an industrial accident occurred at Indian Point 2-involving an operator of a fork lift vehicle.
The vehicle overturned while climbing a hill within the site boundary.
The man died as a result of the accident.
Media interest is expected.
(PNO-79-203) 3.
As a result of an inventory of highly enriched uranium conducted May 18, 1979, at United Nuclear Corporation, Montville, the licensee reported an inventory difference (ID) gain in excess of the calculated limit of error (LEID).
Current causes appear to be the result of errors in the facility i
records.
(PNS-79-67) 4.
The licensee has announced that the Shoreham Nuclear Power Station fuel a
load date has slipped by five months.
The slippage is attributed to l
delays in construction schedule and anticipated delay due to changes to the off gas system and the pipe supports. located in the suppression pool.
The unit was scheduled for fuel load June 1980.
