ALCO DIE 3EL ENGINES NO. 1 AND 2 UNIT 3 SALEM NUC LEAR GENERATING STATION DOCKETS NO. 50-272 AND 50-311 On March 14, 1975 a verbal report was made to NRC Office of Inspection and Enforcement, Region 1, that three heat exchanger heads from No. 1 and one from No. 2 Unit Diesel Engine Lube Oil and Jacket Heat Exchangers for No. 1 and 2 Units, Salem Nuclear Generating Station, were found to be porous. During the hydrostatic testing of the No. 1 Unit Service Water System, water was observed to be leaking from three of the six Emergency Diesel Generator Heat Exchanger water box heads. Examination of the water box heads revealed zones of scattered shrinkage and gas hole porosity with some defects as large as 1/8 to 3/16 inches in diameter.

The heat exchangers in question are size 16078, Type 7-Horizontal heat exchangers manufactured by the Basco Division of American Precision Industries, Inc. This equipment was supplied by the Alco Engines Division of White Industrial Power, Inc. to cool lube oil and jacket water on their Model 18-251 engines.

Following the test on the No. 1 Unit, the field performed a preliminary hydro test of the No. 2 Unit heat exchangers with the result that one head was found to leak slightly for the same reason. Three of the acceptable No. 2 Unit water box heads were used to replace those found defective on the No. 1 Unit.

The three defective water box heads from the No. 1 Unit, along with the one defective head from the No. 2 Unit, were returned to the manufacturer on April 28, 1975 for replacement.

Dr. D.F. Knuth 5-5-75 PSE&G will inve stigate Alco's program and specifications for the procurement of the subject heat exchangers from Basco Division of Ame rican Precision Industrie s , Inc. Alco will be required to audit this supplier and carry out the necessary surveillance to assure that-the replacement heat exchanger heads meet the specified requirements. Implementation of this requirement will be assured by PSE&G Quality Assurance Depart-ment. The replacement heads which will be installed on No. 2 Unit are estimated for delivery by August 1, 1975. The detailed documentation covering this work is available at the Galem site for your future examination.
