NOTIFICATION OF DEFECT; CHARGING/SAFETY INJECTION PUMPS SALEM GENERATING STATION UNIT NOS. 1 AND 2 DOCKET NOS. 50-272 AND 50-311 In accordance with the requirements of 10CFR21, Public Service Electric and Gas Company (PSE&G) forwards the attached report, regarding insufficient welds on the pedestal support alignment lug cup and slide shoe for 11, 12, 21 and 22 Charging/Safety Injection Pumps. These welds may not hold due to physical movement or excessive vibration resulting from a seismic event.

The attached report provides detailed information about the circumstances that led to the reportability determination. PSE&G initially reported this concern on May 16, 1990, in a conference call between R. Binz, F. Schnarr, w. Grau, R. Brown, M. Cirelli, and P. Swetland (NRC Region 1). ATTACHMENT EVALUATION OF DEFICIENCY ON CHARGING/SAFETY INJECTION PUMPS PSE&G had previously determined that the stainless steel cladding on 12 Charging/Safety Injection pump carbon steel casing was damaged. Corrective action involved replacement of the pump casing. While repairs were in progress, a system engineer observed certain welds that did not meet the vendor guidance.

The alignment lug cup and slide shoe pedestal attachment welds are specified on the original vendor documents as 1/2 inch fillet for 360 degrees.

On April 30, 1990, PSE&G contacted the pump vendor (Pacific Pump) to determine the proper weld size. Pacific Pump confirmed the values specified in the original vendor document.

These pumps are supplied by Pacific Pump as part of a complete skid package. Normally, pumps are tack welded for the vendor performance test, with final welding accomplished by the field installer. PSE&G believes that Westinghouse completed the final installation at Salem. If that is true, the responsibility for weld completion lies with Westinghouse. Regardless of responsibility, Pacific Pump or Westinghouse, these welds were deficient on all four (4) Charging/Safety Injection at Salem.

On May 2, 1990, PSE&G and Pacific Pump complete calculations based upon the seismic load criteria furnished by Westinghouse.

The results indicated that the minimum required weld is a 1/4 inch fillet with a total length of 5.8 inches. PSE&G reinspected all Salem pumps to determine if they met the minimum acceptable dimensions. All four (4) pumps were deficient in either weld depth or length. Refer to attached diagrams for actual weld dimensions on 11, 21, and 22 pumps (initial values were not recorded on 12 pump).

CORRECTIVE ACTION Windows were cut in the support pedestals to gain access to the affected areas. All four (4) pumps were welded to at least the minimum required amount. Upon completion of welding activities, the windows were reclosed.
