g On April 24, 1990, Unit 1 was at 100% power. It was discovered that surveillance test SP1042, RTD Bypass Flowmeter Functional Test, had not been performed within the required period. The last day of the 25% grace period i was April 23rd, When discovered, on April 24, 1990, the test was performed immediately and was completed satisfactorily.

The cause of the event was personnel error on the part of the I&C Coordinator who had responsibility for scheduling the performance of SP1042. The I&C Coordinator failed to arrange for pe lormance of the test within the required time frame.

4 EVENT DESCRIPTION .'!

-On April 24,11990, Unit I was at 100% power. The Unit 1 16C Coordinator-discovered that Unit 1 surveillance test SP1042, RTD Bypass Flowmeter <

Functional Test,-was not performed within the required period. .

L SP 1042 is a monthly functional test of the reactor coolant system RTD bypass

. loop.flowmeters. The scheduled due date for completion of SP1042 for the-month of April was April 16, 1990.- Technical Specification Section 4.0 allows surveillance test duo . dates to be adjusted plus or minus 25% - to accommodate normal test schedules. The last day of the 25% grace period for the April

. performance of SP1042 was April 23rd.

The I&C Coordinators, one assigned to each unit, are responsible for the ,

scheduling and performance of the I&C surveillance procedures. The Unit 1.'I&C 1 Coordinator was aware that April 23 was the last day of the 25% grace period ~

-for the completion of SP1042 for the month of April. However, the. Unit 1 I&C d Coordinator was not onsite on April 23, and he failed to assign an I&C- .

technician to perform SP1042 prior to his departure.

The Unit 1 I6C Coordinator noticed on his return to work on April 24, 1990 that SP1042 had not been performed on April 23 as required. The Unit 1 I&C

Coordinator.immediately assigned an I&C teck nician to perform SP1042 and it "

was satisfactorily completed on-April 24, 1390; i CAUSE OF THE EVENT q

Cause-of the event was personnel error on the part of the Unit 1 16C Coordinator who has responsibility for' scheduling surveillance testing for his-group.

The Unit 1 I6C Coordinator failed to arrange for performance of GP1042 within the required time frame.

ANALYSIS OF THE EVENT-This event'is reportable pursuant to 10 CFR'Part 50.73(a)(2)(1)(B). Technical j Specification Table TS.4.1-1 Item 32 requires monthly testing of the RTD Bypass Flowmeters. The performance of SP1042 for the month of March .was satisfactorily completed within the required time period. The results of the ,

April performance of SP1042 were also satisfactory, though the test was '

performed one day late. Therefore, it is believed that RTD Bypass Flowmetsrs-  ;

were operable throughout the period. Health and safety of the public_were.  ;

unaffected. 1 CORRECTIVE ACTION U i

SP 1042 was satisfactorily completed immediately upon discovery that it had not been completed within the required time period.

As a result of this event, the two I6C Coordinators'(one for each unit) will f

-perform' daily reviews of each other's surveillance schedules. This should provide increased assurance that surveillance procedures will be. scheduled for j performance within the required time periods. '

PREVIOUS SIMIIAR EVENTS There have been several instances of missed surveillance tests over the past several years; the most recent event was reported as Unit 1 LER 89-014.
