Gentlemen:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction; Final Report 50-3 Colt Emergency Diesel Generators As required by Title 10, Code of Federal Regulations Part 50, Paragraph 55(e), Northeast Nuclear Energy Company (NNECO) reported two (2) poten-tial significant deficiencies in the construction of Millstone Unit No. 3 in Reference (1). These were reported because diesels similar to

> the ones being supplied for Unit No. 3 experienced problems which could have been generic in nature. The reported significant deficiencies specifically resulted because of the following.

o Failure of crankshaft thrust bearing - A failure of the crankshaft thrust bearing occurred on one of two Colt diesels at South Carolina i Electric & Gas Company's Summer Station. Preliminary evaluation i revealed the most likely cause or failure to be the omission of l plugs in the oil supply system for the bearing, o Failure of crankshaft thrust bearing - A failure of the crankshaft thrust bearing has occurred during shop tests of the Number 4 diesel generator set for the SNUPPS contract. It was determined that the detail drawings provide inadequate clearance between the thrust bearing halves to allow for thermal expansion.

, Since our interim report Colt Industries has provided documentation to l the NRC, see Attachments (1) and (2), closing out these potential signi-ficant deficiencies.

In addition to the above items, your Resident Inspector indicated in a meeting on November 24, 1981 with our Mr. B. L. Carlson, Assistant Project Engineer and Mr. J. M. Powere, Licensing Engineer, that one other open item existed concerning the Emergency Diesel Generators.

l That item was identified as a result of the following incident:

o Flared Fuel Lines lk". During the 300 start test of the Beaver Valley Colt diesels, a flared connection in the ASME III CL 3 fuel return piping was observed to be leaking. Examination of the tube showed a crack at the base of the flare. The failure was determined to be as a result of a stress riser in the tube which was caused by flaring with a flaring tool not having a chamfer or radius on the female portion of the tool.

The Millstone Unit No. 3 Emergency Diesel Generators have been inspected and all 1k" flared fuel lines were replaced, as documented in a letter from Colt Industries to the NRC dated March 21, 1979, see Attachment (3).

This letter constitutes our final report closing out all items related to the Colt Diesel Generators. We trust the above information satis-factorily responds to your concerns.

lw Gentlemen: -

Our June 23,1978 Part 21 Report indicated that the 1-1/4" flared fuel lines on the two Diesel Generators for Millstone III, which were in storage at our plant, would be inspected prior to shipment.

All 1-1/4" flared fuel lines on these two units have been inspected and replaced with lines meeting the fitting manufacturer's recommendaticn for flare 0.D. and radius.

The units are now free of the potential defect reported.
