Dear Dr. Murley:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Accordance with 10CFR50.55(e):

480V Gould MCC - Defective Starter Contact Carrier (SD-89)

In an October 14, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. 3. Young, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 in accordance with 10CFR50.55(e).

The potential significant deficiency involves Telemecanique (formerly Gould)

Class A821 and A822 starters manufactured between May 11,1981 and March 12, 1982, which are used in our Category IE Motor Control Centers (MCC). The defective starter contact carrier assemblies bind, causing starter failure and therefore failure of the associated motor to start.

This deficiency was previously reported in June,1982; however, at that time, the deficiency was identified by the vendor to be limited in scope to eight (8) MCCs based en the shipping date as noted in Reference (1). That deficiency has since been closed af ter completion of inspections of those MCCs. This problem is being re-reported because the above stated findings and subsequent investigation has confirmed that the subject contact carrier assemblies are located in MCCs other than those originally reported.

To eliminate this problem, all size 1 and 2 starters are being inspected for the subject contact carrier assemblies. The vendor has supplied retrofit kits and field service assistance as needed to perform inspections and replace any defective contact carrier assemblies. The inspection and replacement of any defective contact carrier assemblies will be completed for all equipment prior to that equipment being required to be Operable by the Millstone Unit No. 3 Technical Specifications.
