Dear Dr. Murley:
Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction Nonconforming Terminations in CVI Corporation Equipment (5D-75)
In a February 13, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. K. McCarthy, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).
As noted in Reference (1), the potential significant deficiency involved improper terminations found in equipment supplied by CVI Corporation.
Termination deficiencies include crimps which were unacceptable per vendor's Instructions, terminations crimped with the wrong tools, multiple conductors crimped in one lug, and incorrect lug size used.
In Reference (2) NNECO determined that this condition represented a significant deficiency in the final design of Millstone Unit No. 3.

2-NNECO also indicated in Reference (2) that a 100% inspection of the lugs in the two remaining CVI panels (3 HVC*PNLFLT 1 A and IB) would be performed.
Reference (3) and (4) provided status concerning our efforts to complete the lug inspection. NNECO has completed a 100% inspection of the lugs in the two remaining CVI panels. All terminations that were found to be deficient were repaired or replaced.
This hformation is documented in accordance with NNECO procedures. As such, we consider this to be our final report for SD-75.
This final report is being provided on November 12, rather than November 8, as discussed with your Resident Inspector, Mr. T. Rebelowski.
