The deficiency could, during an accident condition, result in degradation or loss in function of safety related systems in which the piping may have been installed.

Attached is an interim report on the subject condition. An updated or final report will be provided by November 20, 1981.

ATTACHMENT 1 INTERIM REPORT WASHINGTON PUBLIC POWER SUPPLY SYSTEM DOCKET NO. 50-397 LICENSES NO. CPPR-93 POSSIBLE INSTALLATION OF DEFECTIVE 2" SCHEDULE 80 PIPE Nature of the Problem The potential problem involves the installation of 2" Schedule 80 Piping which appears to De seamed and have linear surface imperfections. These defects are in Class I systems which could result in failure of the systems to perform their required safety functions.

Approach to the Problem Random samples of the three (3) different heat numbers of pipe were given to Burns and Roe Welding Engineer for evaluation.

The results of this evaluation found that no evidence of welds er seams were observed. As such the pipe in question (heat no.'s HT-N12476, N12477, and J710449) is seamless 2" Schedule 80 as specified.

It has been determined that further inspection and testing must be conducted to resolve the linear surface imperfections of the 2" Schedule 80 Pipe.

Status of Proposed Resolution Select five (5) samples of pipe of eacii heat number with rejectionable indications on the surface 2" Schedule 80 Pipe.

Of the samples selected three (3) of each heat number with the worst surface defects will be tested.

1. Cut pipe at surface imperfection indications.
2. Map any surface imperfection indications in pipe.
3. Prepare and weld pipe.
4. Bend pipe as per ASME SA 106.
5. Hydrostatically test pipe to 2500 PSIG.
6. Cross section pipe in heat affected zone to identify if surface imperfection indications have grown after hydrostatic testing, welding and bending has been performed.

Page 2 Reason Why - Report Will Be Delayed The final report will be delayed until testing on the three heats of pipe in question have been completed. These tests will determine if imptefections exist which could cause pipe failure while in service.

Projected Completion of Corrective Action and Report It is anticipated that the final report identifying the final resolution of this issue and any associated corrective action will be provided by November 20, 1981.
