Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction: Main Control Board Welds (SD-36)

In an April 13, 1983, telephone conversation between your Mr. T. Elsasser and l our Mr. D. B. Vail, Northeast Nuclear Energy Company (NNECO) reported a l potential significant deficiency in the construction of Millstone Unit No. 3 as

! required by Title 10 Code of Federal Regulations Part 50, Paragraph 55(e). The potential significant deficiency involves inadequate welds on main control board panels supplied by Reliance Electric Company.

During construction operations to replace the 3" x 6" x 1/2" vendor installed seismic anchor plates with larger ones, it was discovered that when the bottom weld was ground away, the plate became unattached at the upper weld seam with minimal manual ef fort.

In addition, numerous welds on panel skin stiffeners appear to have cracked and broken free on the main control board.

Problems with Reliance main control board panels identified in NRC I.E.

Information Notices No. 82-34 and No. 82-34 Rev.1 indicate that deficiencies exist in the construction of the main control board and possibly the implementation of Reliance Electric Company's Quality Assurance Program.

4 A meeting of representatives from Reliance Electric Company, Stone & Webster Engineering Corporation, and Northeast Nuclear Energy Company is scheduled for May 17, 1983 to determine the corrective actions necessary to resolve this l problem and also the completion date for resolution.
