Dear Mr. O'Reilly:

On January 13, 1981, a potential significant deficiency was reported to John Rausch of the Nuclear Regulatory Co: mission Region II office. The item involved a valve actuator that was not fully assembled. This apparently caused the valve to leak during testing.

Scme reports pertaining to the investigation have been

. received, but at this time no conclusions can be drawn. Reports show the valve was inspected and it was intact when it arrived on site. An effort is being made to determine how the valve actuator could have been unassembled or damaged during plant operations. A final report is expected by March 13, 1981.
