Dear Mr. O'Reilly:

On February 17, 1980, a substantial safety hazard as defined by 10CFR 21 was reported to Mr. Virgil Brownlee at the Nuclear Regulatory Commission, Region II office. The item involved the seismic qualification of Agastat relays in vendor supplied equipment. The designer became suspicious of the seismic qualification of the relays while trying to qualify them for another panel. The vendor had addressed seismic qualification, but there was not enough information in the qualification report to verify qualification.

Details are given on the attachment.

The item is being reported under our 10CFR 50.55(c) procedure.

This is considered a final report on the matter.

Nature of Defect, Substantial Safety Hazard Created, and Evaluation The available seismic qualification data on the Agastat relays installed in this equipment did not clearly demonstrate that the relays would time properly in a seismic event. This could result in improper timing and either f.nitiating trips too soon or delaying for too long in a seismic event.
5. Date Information of Defect was obtained -

December 22, 1980

6. Number and Location of Defect There were four relays in the undervoltage/underfrequency panel and six in the medium voltage switchgear.
7. Corrective Action The Agastat relays in the medium voltage switchgear were removed due to a previous modification. The relays in the undervoltage/underfrequency panel are being moved to a panel with a lower response spectrum to which the relays are qualified.
8. Advice to Purchasers or Licensees None.
