I I I On May 13, 1993, at 1418 hours0.0164 days <br />0.394 hours <br />0.00234 weeks <br />5.39549e-4 months <br />, a fire watch reported a Service Water (SW) leak in Mechanical Equipment Room (MER) four. The leak was identified to be from a two inch backwash supply line [EIIS-BI-PSP] to a Self Cleaning Continuous Backwash Strainer [EIIS-BI-STR], (1-VS-S-lA) in one of two SW flow paths to the Main Control Room (MCR) Chillers. Technical Specification (TS) 3.14.C. requires two operable SW flow paths to one operating and one operable MCR Chiller. The line and strainer associated with the leak were declared inoperable.

At the time of this event, TS 3.14.C had no action statement for an inoperable SW flow path, therefore a six hour LCO to Hot Shutdown was entered for Units 1 and 2 at 1418 hours0.0164 days <br />0.394 hours <br />0.00234 weeks <br />5.39549e-4 months <br /> per TS 3.0.1.* An alternate flow path was placed into service and the action statement was exited at 1733 hours0.0201 days <br />0.481 hours <br />0.00287 weeks <br />6.594065e-4 months <br />.

The backwash supply line was repaired following issuance of a change to TS 3.14.C.

The TS change provided a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> LCO allowing one flow path to be out of service for maintenance. At the time of the event the leakage was evaluated to be manageable with regard to design basis requirements. It is therefore concluded that no safety consequences resulted from the event.

1.0 DESCRIPTION

OF THE EVENT On May 13, 1993, at 1418 hours0.0164 days <br />0.394 hours <br />0.00234 weeks <br />5.39549e-4 months <br />, Unit 1 was at 100% power and Unit 2 was at 92% power. A fire watch reported a SW leak in Mechanical Equipment Room (MER) four. The leak was identified to be from a two inch backwash supply line to a Self Cleaning Continuous Backwash Strainer (1-VS-S-lA). The strainer is in one of two Service Water (SW) flow paths to the Main Control Room (MCR) Chillers required by Technical Specification (TS) 3.14.C. The SW flow path containing the two inch backwash supply line is ASME Code Class 3. The two inch backwash supply and the 1-VS-S-lA strainer were' declared inoperable.

Surry Power Station, Unit 1 TEXT (ff man, apace i& n,quirtKJ, UM t1dditional NRC Form 356A 'a) 117) 2.0 SIGNIFICANT SAFETY CONSEQUENCES AND IMPLICATIONS The SW System is designed for the removal of heat resulting from the operation of various systems and components. Per TS**a.14.C one operating and one operable SW flow path to one operating and one operable MCR Chiller are required. Adequate SW flow was available to meet demands of the system. No operating parameters were affected on the MCR Chillers or Charging Pump SW pumps. The operating MCR Chillers and Charging Pump SW pumps remained in service.

The water inleakage was manageable, and did not affect operation of supported equipment. It was concluded that no safety consequences resulted from the event.

3.0 CAUSE OF THE EVENT The event was caused by a component failure of the two inch backwash supply line to the 1-VS-S-lA strainer. This two inch pipe was constructed of schedule 80 carbon steel. Flow through this path consists of brackish water. It was determined that the failure was caused by general corrosion of the pipe. The corrosion resulted in thinning of the pipe wall and eventually resulted in the through wall leak.

4.0 IMMEDIATE CORRECTI\7E ACTION

Operator response to the event placed the Duplex Strainer [EIIS-BI-STR], (1-SW-S-10) in service bypassing* the 1-VS-S-lA strainer.

Maintenance installed a temporary patch on the two inch backwash supply line and an oper~tor was assigned to monitor the line. The systems and components being serviced by the affected SW lines were routinely monitored.

5.0 ADDITIONAL CORRECTIVE ACTION{Sl Since the affected piping was an ASME Code Class 3 low energy system, repair strategies were evaluated using'Generic Letters 91-18 "Resolution of Degraded and Non Conforming Conditions" and 90-05 "Guidance for Performing Temporary Non-Code Repair of ASME Code Class 1, 2, and 3 Piping." 6.0 ACTIONS TO PREVENf RECURRENCE The two inch backwash supply line and the 1-VS-S-lA strainer and associated isolation valves were replaced. The backwash supply line was replaced with a flexible line. This was completed after the receipt of a change to TS. TS 3.14.C.1 was issued 5/17/93 and allows one SW flow path to be inoperable for 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. This change was submitted September 4, 1992 as a corrective action resulting from LER Sl 009.

TS change 3.14.Cl, issued 5/17/93, provides a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> LCO to permit maintenance activities anticipated on the SW flow paths to the MCR Chillers.
