On March 18, 1992, at 1356 hours0.0157 days <br />0.377 hours <br />0.00224 weeks <br />5.15958e-4 months <br />, with Unit 1 at refueling shutdown and Unit 2 at 100% power, Emergency Diesel Generator Number 3 (EDG 3) automatically started upon receipt of a Unit 1 4160 volt emergency bus 1J undervoltage (UV) signal. Th~ UV signal also caused annunciator K-H-3, "BUS 1J UNDER VOLT", to alarm in the control room. The control room responded by checking the emergency bus 1J voltage and determined that the _voltage was normal. EDG 3 was also verified to be operating properly. An assistant shift supervisor was dispatched to the emergency bus 1J UV relay panel to investigate the cause of the UV signal. The assistant shift supervisor discovered that contract electricians were working in the subject panel and determined, based on this observation and the absence of other indications of an actual UV condition, that an UV relay had been bumped, resulting in the UV signal. EDG 3 was secured and placed in "Auto" at 1428 hours0.0165 days <br />0.397 hours <br />0.00236 weeks <br />5.43354e-4 months <br />.* This event was caused by a personnel error in that a contract electrician inadvertently bumped an emergency bus 1J UV relay while working in the emergency bus 1J UV relay panel. A Human Performance Enhancement Syst~m evaluation is being conducted to determine how this event may have been prevented. This event is being reported, pursuant to 10 CFR 50.73(a)(2)(iv)',

as an unplanned Engineered Safety Feature (ESF) actuation.

1.0 DESCRIPTION

OF THE EVENT On March 18, 1992, at 1356 hours0.0157 days <br />0.377 hours <br />0.00224 weeks <br />5.15958e-4 months <br />, with Unit 1 at refueling shutdown and Unit 2 at 100% power, Emergency Diesel Generator Number 3 (EDG 3) {EIIS-EK,DG}

automatically started upon receipt of a Unit 1 4160 volt emergency bus {EIIS-EB,BU} 1J undervoltage (UV) signal.

The UV signal also caused annunciator {EIIS-IB,ANN} K-H-3, "BUS 1J UNDER VOLT", to alarm in the control room. The control room responded by checking the emergency bus 1J voltage and Qetermined that the voltage was normal.

  • An assistant shift supervisor was dispatched to the emergency bus _1 J UV relay panel

{EIIS-JE,PL} to investigate the cause of the UV signal. The assistant shift supervisor discovered that contract electricians were working in the subject panel* and determined, based on this observation and the absence of other indications of. an actual UV condition, -that an UV relay {EIIS-JE,27} had been bumped, resulting in the UV signal.

EOG 3 reached the required nominal speed of 900 RPM, as designed, but did not load on emergency buses 1J or 2J since an actual UV condition _did not exist. EOG 3 was secured and placed in "Auto" at 1428. hours ..

A non-emergency four~hour report, pursuant to 1o CFR 50.?2(b)(2)(ii), was*

made to the Nuclear Regulatory Commission at 1647 hours0.0191 days <br />0.458 hours <br />0.00272 weeks <br />6.266835e-4 months <br /> on March 18, 1992..

This event is being reported, pursuant to 1o CFR 50.73(a)(2)(iv), as an unplanned Engineered Safety Feature (ESF) actuation.

2.0 SIGNIFICANT SAFETY CONSEQUENCES AND IMPLICATIONS This event resulted in no negative safety consequences or implications since an

.actual emergency bus UV condition *did not exist. In addition, EOG 3 automatically started, as designed, and would have provided power to the appropriate buses in the event of a valid UV condition. Therefore, the health and safety of the public were not affect~d by this event.

3.0 CAUSE OF THE EVENT This ESF actuation resulted from an inadvertent emergency bus 1J UV signal which was caused by a personnel error. Upon investigation, it was concluded that the UV signal was initiated when a contract electrician inadvertently bumped an emergency bus 1J UV relay while performing unrelated work in the emergency bus 1J UV relay panel.

4. 0 IMMEDIATE CORRECTIVE ACTIONCSl The control room responded by checking the emergency bus 1J voltage and determined that the voltage was normal. EDG 3 was also verified to be operating properly. Control_ Operations personnel investigated the cause of the UV signal and determined that no UV or degraded voltage condition was present on emergency bus 1J that would require an automatic start of EDG 3.
