At the time of the event, the 11 C11 chiller was operable and the "A" chiller was out of service. This condition is contrary to Technical Specification 3.14 which requires that one MCR/ESGR chiller be operating and another be operable whenever fuel is in the reactor core. Administrative controls have been established to ensure the chilled water and service water valves remain in the proper position.

1 I 1.0 Description of the Event On June 13, 1989 at 0949 hours0.011 days <br />0.264 hours <br />0.00157 weeks <br />3.610945e-4 months <br />, with Units 1 and 2 in cold shutdown, it was determined that the operating Main Control Room/Emergency Switchgear Room (MCR/ESGR) chiller, l-VS-E-4B, was inoperable due to degraded performance. At the time of the event, the 11 c11 chiller was operable and the 11 A11 chiller was out of service. This condition is contrary to Technical Specification 3.14. It was determined that an improper valve lineup existed for the chilled water and service water systems for the 11 B11 chiller. This resulted in actuation of the refrigerant relief valve and degradation of chiller performance. The 11 c11 chiller was placed in operation at 1028 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91154e-4 months <br />. The 11 11 B chiller was recharged with its chilled water and service water flows properly balanced, and returned to service at 1125 hours0.013 days <br />0.313 hours <br />0.00186 weeks <br />4.280625e-4 months <br />.

2.0 Safety Consequences and Implications The MCR/ESGR air conditioning system consists of three air conditioning chiller units. The design temperature of the MCR/ESGR spaces is from 40 degrees Fahrenheit to 120 degrees Fahrenheit.

During this event, the MCR/ESGR air handling units remained operating, and there was no significant increase in ambient temperature of the MCR/ESGR areas.

Therefore, the health and safety of the public were not affected.

3.0 Cause Insufficient administrative control of the valve lineup for the chilled water and service water (SW)

(EIIS-BI) systems for the 11 B11 chiller was the cause of the event. This resulted in a flow imbalance that resulted in inefficient heat transfer between the two systems, overpressurizing the chiller's refrigerant.

This actuated the chiller's relief valve causing a loss of refrigerant and degradation of the chiller heat removal capability.

The 11 11 B chiller was returned to service at 1125 hours0.013 days <br />0.313 hours <br />0.00186 weeks <br />4.280625e-4 months <br />, and the c chiller was removed from operation and 11 11 maintained in an operable status at 1212 hours0.014 days <br />0.337 hours <br />0.002 weeks <br />4.61166e-4 months <br />.

6.0 Action(s) Taken to Prevent Recurrence Administrative controls have been established to ensure the valves remain in the proper position.
