  • DESCRIPTION OF EVENT:

During the review of PT-270, Heat Tracing Circuit Monthly Check, it was discovered that Heat.Tracing Circuits 2A., B & C (Panels 8 and 9) for. th~

Unit 1 Boric Acid Filter System has been tagged out of service since 2/11/80. This. is contrary to T.S. 3.2.a.5 and report_able per T.S. _6.6.2.a.2.

2. PROBABLE CONSEQUENCES & STATUS OF REDUNDANT SYSTEMS:

Normal operation verified that the required flow paths were operable, therefore, the health and safety of the public were .not affected.

3. CAUSE:

In performing the tag-out necessary to effect repairs on circuit 24B (Panels B & C) Unit 1 Boric Acid Filter Drain Line,. all three circuits were inadvertently tagged out of service.

4. IMMEDIATE CORRECTIVE ACTION:

The two circuits of heat tracing that are required by Tech. Specs. were immediately energized and flow verified.

5. SUBSEQUENT CORRECTIVE ACTION:

Prior to unit startup, all circuits will be repaired and verified operable~ The boric acid leak will be repaired.

  • ACTION TAKEN TO PREVENT RECURRENCE:

The program of review for tagging out of electrical equipment.has been stressed to the people involved and no further action is deemed necessary.

7. GENERIC IMPLICATIONS:

A task force has been established to study the total spectrum of the heat tracing system in an attempt to discern present problems and recommended solutions which will eliminate the failures experienced. While this study is in progress, a new type of Heat Tracing Tape is being used which has thus far proven resistant to failure.
