Surry Power Station, Unit No. 1 Docket No: 50-280 Report No: 79-028/03L-O Event Date: 10-03-79 Type of Report: Safety Related Anchor Bolt Deficiencies

1. Description of Event:

During the performance of Special Test 75, Anchor Bolt Test and inspection, in accordance with NRC F~E. Bulletin 79-02, 10 of 209 bolts inspected failed to meet with the specified Acceptance Criteria. This is reportable pur~uant to Technical *specification 6.6.2.b.2. Other anchor bolt deficiencies were noted, evaluated, and determined to meet the specified Acceptance Criteria thereby not affecting the integrity of the associated supports.

2. Probable Consequences and Status of Redundant Systems:

Since more than 95% of the anchor bolts met or exceeded the specified Acceptance Criteria the overall integrity of the supports involving safety related systems was not jeopardized .. Therefore, the health and safety of the general public were not affected.

3. Cause:

It appears that the anchor bolts were not properly installed at the time of initial construction.

4. Immediate Corrective Action:

All deficiencies noted were evaluated by Engineering and corrected as required.

5. Subsequent Corrective Actions:

A continuing program of inspection and repair is being developed and implemented.

6. Actions Taken to Prevent Recurrence:

Full compliance with NRC I.E. Bulletin 79-02 will preclude a recurrence.

Unit No. 2 is presently being inspected and evaluated,
