1. Description of Event:

With the unit at cold shutdown, valve MOV-SW-203D failed to operate electrically during PT-8.SA (Consequence Limiting Safeguards functional test).

This is contrary to Technical Specification 3.14.A.6 and is reportable per Technical Specification 6.6.2.b(3).

2. Probable Consequences and Status of Redundant Systems:

Valve MOV-SW-203D provides one flow path of service water to the Recirculation Spray Heat Exchangers. However, 100% flow capability existed through M0,V-SW-203A and B which were demonstrated to operate properly during PT-8.SA, therefore, the health and safety of the public were not affected.

3. Cause:

MOV-SW-203D failed to operate during PT-8.SA testing because of a mechanical binding of the valve.

4. Immediate Corrective Action:

The immediate corrective action was to attempt to manually operate the valve. The valve would still not cycle, therefore PT-8.SA testing was continued.

5. Subsequent Corrective Action:

MOV-SW-203D has been completely dismantled, inspected, cleaned, and lubricated. A bearing in the limitorque operator was replaced.

6. Future Corrective Action:

There is not any future corrective action deemed necessary at this time.

7. Generic Imulications:

None
