REPORTABLE DEFICIENCY IN WIRING INTERFACE FOR SAFETY-RELATED VALVE MOTOR OPERATORS NATURE OF POTENTIAL PROBLEM A review of a vendor drawing submittal in regards to motor operated valves from the Borg-Warner Corporation was conducted in October of 1980. During this review, Burns and Roe Engineering discovered a mismatch between the connection/wiring diagrams for RCIC-V-69, RCIC-V-110 and RCIC-V-113, and vendor wiring diagrams. Furthermore, the accompanying motor data for RCIC-V-69 indicated a voltage rating of 125 VDC, whereas the contract specification indicated a voltage of 250 VDC which is a design requirement.

Drawing E-540, Sheets 1 and 2, indicate cable connections direct to the limit switch terminals and limit switch interconnections to be done by the electrical construction contractor. The vendor wiring diagram shows prewired connections of the limit switches to a terminal block and between selected limit switches.

A lid i y the aforementioned valves require terminal blocks to be provided for connection of all external cabling. Apparently, this requirement was not considered in the design stage of Drawing E-540 nor was the vendor drawing submittal used as basis for the construction drawing.

APPROACH TO AND STATUS OF PROBLEM RESOLUTION Burns and Roe is evaluating the scope, cause and reportability of the problem. The connection/wiring diagram for each motor operated valve under the particular contract, Contract 215, will be reviewed against the vendor's wiring diagram. Changes in the construction drawings will be made as needed and the installed wiring r eterminated accordingly.

The Contract 215 contractor has been informed of the voltage problem of RCIC-V-69. A field inspection at the receiving warehouse will be made to determine if the motor, as shipped, is in fact deficient. If so, the motor operator will be replaced. The contractor's procurement procedure will be reviewed to determine adequacy and compliance to the contract specifications.

Attachment Page 2 of 2 PROJECTED COMPLETION OF FINAL REPORT AND CORRECTIYE ACTION There are around 45 safety-related motor operated valves in Contract 215. The design problem could also apply to non-safety-related valves and valves procured under other contracts should the deficiency be generic to the design procedure. Hence, addi-tional time is required to complete the review process. A follow-up report will'e submitted by March 18, 1981.
