MPPSS NUCLEAR PROJECT NO. 2 REPORTABLE DEFICIENCY AND CORRECTIVE ACTION FOR IYiPROPER INSTALLATION OF CONCRETE ANCHORS ilashington Public Power Supply System Docket No. 450-397 License No. 8CPPR-93 NATURE OF DEFICIENCY Improper installation (under expansion) of concrete anchors.

SAFETY IMPLICATION The improper installation, if left uncorrected, could cause these concrete anchors to fail. Subsequently, this could result in failure of safety related equipment during the operational life time of the Nuclear Power Plant.

CORRECTIVE ACTION Procedures of affected contracts have been reviewed. Engineering change orders, updating the installation and inspection requirements, including a retrofit program, have been issued to all contractors whose procedures did not assure proper installation. Installed inserts will be tested/in-spected to assure compliance with requirements or will be replaced.
