TENNESSEE VALLEY AUTHORITY (TVA) - SEQUOYAH NUCLEAR PLANT (SQN) - NPDES PERMIT NO. TN0026450 - DISCHARGE MONITORING REPORT (DMR) FOR July 2014 Enclosed is the July 2014 Discharge Monitoring Report for Sequoyah Nuclear Plant. The turbine building sump (TBS) was discharged directly to the yard drainage pond during the reporting period and was monitored in accordance with the narrative condition found in Part 1.A.2 of the subject permit. The TBS monitoring data is enclosed as Attachment 1.

Also, during the reporting period, Pace Labs was unable to provide analytical results for the July 21 total suspended solids (TSS) sample due to a laboratory error. A back-up sample was sent to Pace Labs, and the results are included in the DMR for that date. A letter of explanation was provided by Pace Labs and is enclosed as Attachment 2. At no time has there been any observed threat to public drinking supplies, to human health, or the environment. TVA Sequoyah Nuclear Plant NPDES Permit Number TN0026450 Attachment 1 Turbine Building Sump Monitoring Data The turbine building sump was discharged directly to the yard drainage pond from 7/15/2014 to 7/22/2014. During this period, the turbine building sump was monitored in accordance with the narrative condition found in Part 1.A.2 of the NPDES Permit TN0026450. There are no permit limits applicable at this monitoring point, which flows to the yard drainage pond, mixes with other flows in the diffuser pond, then discharges to the Tennessee River at Outfall 101.

  • Analytical data for samples collected on 7/19/2014 are not representative of the nature of the discharge based on visual observations during time of collection. In addition, samples taken before and after this date are well below the daily allowable maximum.

However, a thorough investigation did not identify an apparent cause.

    • Analytical data for back-up sample collected on 7/21/2014 is included in the calculations. The hold time for this sample was exceeded due to the laboratory event described in Attachment 2.

Dear Ms. Garland:

I am writing in regards to the investigation into why the below sample was not able to be calculated for Total Suspended Solids (TSS). Here is a summary of the investigation into the Total Suspended Solids sample.

Your sample SQN-TBS (92208297) was received by Pace on July 25, 2014. The sample was received by our Pace - Asheville laboratory for the requested analysis. Due to an electronic program malfunction, we were not able to recover the initial filter weight needed to calculate a result for this sample. The laboratory did not have any other samples to utilize for reanalysis. Therefore, we could not provide you with the TSS analysis as requested. We apologize for the inconvenience this has caused.

As a result of this error, our IT department is currently figuring out what happened so we can prevent any future occurrences of this type. We are also implementing a new procedure to have the analysts print out the initial filter weights from the electronic program so that we have them in case there is ever another incident and be able to calculate out the TSS results.
