Request for Additional Information - Palisades - 2012 SG Inspection Report - ME9924 Date: Tuesday, February 19, 2013 1:35:00 PM By letter dated November 01, 2012 (Agencywide Documents Access and Management System (ADAMS) Accession No. ML12306A534), Entergy Nuclear Operations, Inc. (the licensee) submitted information pertaining to their 2012 steam generator tube inspections at the Palisades Nuclear Plant (Palisades). In addition to this report, the U.S. Nuclear Regulatory Commission (NRC) staff summarized conference calls about the 2012 steam generator tube inspections at Palisades in letters dated June 21, 2012 (ADAMS Accession No. ML12165A389), and July 17, 2012 (ADAMS Accession No. ML12188A578). In order to complete its review, the NRC staff requests the following additional information:

Please arrange a teleconference with the NRC staff to discuss this information.

1. Page 3 states that the Palisades replacement steam generators (RSGs) are in their 22nd sequential inspection period. Table A shows that the RSGs are in their 4 th sequential inspection period. Please clarify.
2. Table 2 indicates that historical foreign object wear is an active damage mechanism in SG E-50B only, and does not refer to foreign object wear being currently active in either SG E-50A or E-50B. However, Tables 5 and 6 show that two tubes in SG E-50A were plugged in the 2012 refueling outage due to loose part wear. Please clarify.
3. Please define the acronyms BED and BEL, used in the Sizing column of Table 3.
4. Please discuss the results of the secondary side steam drum and feedring visual inspections performed in SG B.
5. Please discuss if any tube-to-tube wear was detected this outage.
6. Please discuss any efforts underway (and schedules) to address the NRC staffs comments in its July 17, 2012 letter.
7. Please discuss why the tube in Row 51 Column 150 in SG A was preventively plugged.
8. Please discuss whether any cracks were detected at historical wear locations, eggcrates, diagonal bars, or vertical straps. Please confirm that all new wear indications were inspected with a rotating probe to characterize the indications.
