SYNOPSls
.
On August. 8,1984, a member of the NRC Technical Review Team (TRT) at the Comanche Peak Steam Electric Station (CPSES) was unable to locate an electrician's Electrical Test Group (ETG) Indoctrination form (and/or readinglist). This form, which is not required by station procedures or NRC regulations, was missing from the Texas Utilities Generating
,
Company (TUGCO) and Brown & Root, Inc. (B&R) training record files. A TUGC0 start-up specialist subsequently furnished the form to the TRT representative who then questioned its authenticity.
Therefore, the '
start-up specialist contacted a B&R general foreman who had made the form available to.him (the start-up specialist).
The general foreman explained that he had arranged to have the missing' document " regenerated."
In view of these circumstances, an investigation was conducted to determine whether TUGC0 and/or B&R personnel had. attempted to deceive the NRC by misrepresenting a missing training record which had been
recreated and predated.
When interviewed, the general foreman acknowl-
,
edged that since he was unable to locate the indoctrination form re-quested by the NRC, he instructed the electrician to complete a new form and include a date which corresponded with the approximate'date the
!
training had been received.
He stated he independently decided to have
the form regener'ated, and he had not intended to deceive the NRC since the electrician had received the indoctrination training indicated on the fom and the document is not required by B&R procedures or the NRC.

at the general foreman's direction; and advised that he previously had

received the indoctrination training 'as reflected on the recreated form.

Inquiries and records also verified

that the indoctrination form is not a required document by B&R or TUGC0 written procedures or. the NRC..There was no evidence of other levels of l
management sanctioning or having knowledge of this matter.
