SUBJECT:

POSSIBLE FALSIFICATION OF FUEL P0OL TRAVELERS The following scenario relates to the sign-off of Traveler #591, and is based on the signatures and dates on the document. The scenario is speculative, but the implications are that James W. Cole and Sam Bell were colluding in falsify-i ing sign off of inspection hold points on fuel pool travelers. The falsifica-i tion, if true, would be in signing off inspection activities that were not I

conducted.

The scenario, associated with the attached traveler #591, follows:

o About 1/10/82 Cole noticed that #591 was due final QC inspection. At this point only one page existed and Hold Points 7 and 8 had SAT results with the 12/4/81 and 1/15/82 dates pre-entered (the common practice at Comanche Peak) and only the inspectors' signatures remained to be filled in.

o Cole added the second page (page 2 of 2), since this would be needed to complete the inspection record, then sent both' sheets to his pal Bell to complete the V.T. and P.T. inspection.

o Bell filled in the P.T. part, backdating it to 11/30/81, and changed the step 7 V.T date to 11/30/81, to give Cole a few days to complete the vacuum box test before the originally scheduled 12/4/81 date, returning the form to Cole by 1/13/82.

o Cole made some corrections in procedure numbers on Bell's P.T.

sign-off on page 2, then filled in the vacuum box test data backdating the entry to 12/4/81; but in entering the date he made a revealing blunder; he put 12/4/82 (having already programmed himself to write the new year) and then had to correct it to 1981.

o Cole then chan1ed the Hold Point 8 date to the current date 1/13/82, completing the inspection sign-off for #591.

In all likelihood the V.T., P.T., and vacuum box tests were not done. Being generous, one could concede that the V.T. and P.T. may have been done, but it is patently obvious that if the vacuum box test was done at all it certainly was not done on 12/4/81, as the record now indicates.

V. Noonan . In discussions with the lawyers during our SSER review they believe falsification does not belong in our effort but should be assessed by the Office of Investigation. After discussion with A. Vietti we contacted RIV OI, Brooks Griffin, who agreed it should be sent to their office. We, therefore, request that this matter be transferred to RIV OI.

Enclosed is a copy of traveler #591 and a listing of other travelers involving the two specific individuals that we have in our backup files. This would obviously be only a partial list of travelers on which these two were involved.

The scenario, associated with the attached traveler #591, follows:

COMANCHE PEAK SCHEDULE AND ORGANIZATIONAL CHART hva k. --

The purpose of this memorandum is to provide you with the Comanche Peak schequie, which identifies all activities that must be ccmplete prior to the resumption of the ASLB hearings, and the organizational chart for accomplishing these activities (Enclosure 1 and 2, respectively).

Since the Cemanche Peak Technical Review Team (TRT) work as it relates principally to construction at the site is nearly complete, the Comanche Peek Task Force must integrate the TRT findings with all activities necessary to conclude the overall licensing effort. This will include all remaining safety issues relating to design, construction and operation of Comanche Peak that must be addressed in order to resume the ASLB hearings.

All remaining licensing actions required for Comanche Peak will remain under the management of V. Nocnan, Director of the Comanche Peak Project. The enc'losed organizational chart sets forth the arrangement for carrying out the responsibilities for the project. As previously indicated, Mr. Noonan will coordinate these efforts with NRR, IE, and Region IV. In order for V. Noonan to properly carry out the responsibility of completing all matters affecting the licensing of Ccmanche Peak and ensure consistent communication with the applicant and entervenors, all routine licensing correspondence should be prepared for V. Noonan's signature. In addition, he should continue to concur in all Comanche Peak external correspondence prepared by Region IV, such as, inspection reports and enforcement actions.

The organizational chart reflects three major groups: the Engineering Group, headed by L. Shao; the Licensing Grcup, headed by C. Trammell; and the Electrical / Operational Group, headad by J. Calvo and identifies those individuals who are responsible for activities within these groups.

Enclosure 3 identifies those individuals who will be needed on a full or part time basis through the completion of the ASLB bearings, and major areas of responsibility.
