Exit Interview The inspection scope and findings were summarized on June 19, 1981, with those persons indicated in Paragraph 1 above. The following specific items were discussed in detail:
(Closed) IE Bulletin 80-08, Examination of Containment Liner Penetration i
Welds i
(0 pen) Unresolved Item (413, 414/81-04-01) Missing Radiographic Technique Records i
3.
Licensee Actico on Previous Inspection Findings (0 pen) Unresolved Item (413, 414/81-04-01) Missing Radiograpnic Technique Records.
The licensee provided the inspector with a copy o t'
a manual containing all outdated radiographic technique sheets which are maintained
-
in the record storage vault in the corporate office. However, the technique sheet in question, 5-1-J, was not a part of this manual.
The licensee advised that either this technique sheet was not officially issued or it may l
be in a different file in the storage vault. The licensee agreed to search further for the techniqur: sheet in question. Thus, this item remains open
'
until the licensee resolves the RT technique in question.

Unresolved Items Unresolved items were not identified during this inspection.
5.
(Closed) IE Bulleting 80-08 - Examination of Containment Liner Penetration Welds The inspector reviewed design drawings, records and radiographs for several penetration welds of both Units 1 and 2 during an inspection in March 1981.
During this inspection, two discrepancies were noted and identified in report 413, 414/81-04. The discrepancies were resolved as follows:
,
a.
The records for penetrations 142 (H-6871) and 343 (H-6883) indicated that neither radiography nor ultrasonic examination had been performed on the welds.
Further investigation by the licensee disclosed that these were not the flued head welds addressed in the Bulletin. H-6871 and H-6883 are the bellows attachment welds which only require a penetrant examination, which was done. The flued head welds addressed i
in the Bulletin for the penetrations, identified above, are H-6870 and H-6882 respectively. These welds were examined ultrasonically, and as such, comply with applicable requirements.
b.
The radiographs for penetrations 110 (H-6862 - Unit 1) and (H-6866 -
Unit 2) and 309 (H-6868 - Unit 2) showed the weld thickness to be less than the adjacent base metal, and as such, could have violated minimum wall thickness.
During this visit, the inspector and the licensee sighted two of the subject welds and determined that H-6866, and another weld nearby (H-6867), had been contour ground on the inside surface. At the request of the inspector, the licensee ultrasonically examined the adjacent base metal (of these two welds) for wall thick-ness and measured the depth of the contour ground weld with a pit gauge. These measurements determined that the minimum wall thickness had not been violated.
Therefore, the welds in question were considered to be acceptable to the applicable requirements.
Based on the above resolutions, the Bulletin is closed.
No violations or deviations were noted.
6.
Preservice Inspection - Liquid Penetrant Examination
,
The inspector requested B&W to perform a liquid penetrant examination on a safety-related piping weld that would simulate the examination they did for previous inspection at the McGuire site.
This was requested since B&W performed the preservice inspections at McGuire, but liquid penetrant examinations had not been observed at McGuire. Conduct of this examination
will also be identified in the McGuire report 370/81-08.
,
No violations or deviations were noted.
