Exit Interview The inspection scope and findings were summarized on February 15, 1985, with those persons indicated in paragraph 1 above.
The inspector described the areas inspected and discussed in detail the inspection findings listed below.
No dissenting comments were received from the licensee.
Violation 400/85-08-01, Inadequate Design Control for Raceway Separation - paragraph 5.
Unresolved Item 400/85-08-02, Evaluation of Reduced Separation Between Non-Class IE Conduit and Open Class IE Raceway - Paragraph 5.
The licensee did not identify as proprietary any of the materials provided to or reviewed by the inspector during this inspection.

Back round As a result of an OIE CAT inspection conducted during October and November 1984, one item in the electrical ar ea was referred to-Region II for action as a potential enforcement finding.
This item is discussed in Sections II.B.1 and II.B.2 of Inspection Report 50-400/84-41.
Region II assigned an Unresolved Item (URI) number to the finding for identification and tracking purposes, and for further review and investigation.
The evaluation and resolution of this potential problem are as follows:
URI 50-400/84-44-02, Design Control Related to the Separation of Class 1E and Non-class 1E Electrical Raceway Components.
The licensee has committed to meet the Institute of Electrical and Electronic Engineers (IEEE)
Standard 384-1974 in the Final Safety Analysis Report (FSAR).
IEEE Standard 384-1974 titled, "Criteria for the Separation of Class IE Equipment and Circuits" establishes the requirements for the separation and independence of Class IE circuits.
FSAR Section 8.3.1.2.30, paragraph a)
2) (a), specifies the requirements for physical separation of electrical raceways, which states, in part,
"Non-Class IE circuits are routed in separate raceway systems from Class IE circuits.
Non-Class IE raceway are separated from Class IE raceway systems as specified in Section 8.3.1.2.30(b).
Section 8.3.1.2.30(b)
states,
"Where the above separation distance could not be maintained, suitable fire barriers and/or approved protective coating have been utilized or the cables have been installed in enclosed raceway which are suitable for protecting the cables from a fire in adjacent raceway" and
"The minimum separation distance between enclosed raceways or between bar riers and the protected raceway is one inch".
The OIE CAT report 50-400/84-41 identified, in Table II-2, raceway segments which did not have adequate separation between Class IE and Non-Class IE raceway.
The table also identified several instances of separation deficiencies between redundant Class IE division wiring and raceways.
The inspector has examined each of the above concerns separately.

Separation Deficiencies between Class IE and Non-Class IE Raceways Resolution The inspector randomly selected approximately 50% of the raceway deficiencies identified by the CAT inspectors for inspection.
The examination confirmed deficiencies on all raceways inspected.
Based on the sample, the inspector concluded that all CAT findings were valid.
The Region II inspector also identified several deficiencies in addition to those identified by CAT:

P1803-SB 17058K, 17058H, 10731E, 17065S P1816-SB 16094Q The inspector examined Field Change Request (FCR)
number E-1304 which was approved by the licensee's Harris Plant Engineering on February 27, 1984.
The inspector determined that the design change detailed in FCR-E-1304 allowed Non-Class IE conduit to be separated, from open Class IE raceway, by one inch.
This change did not meet the commit-ment to the IEEE Standard 384-1974, which requires that either both raceways be enclosed or an approved fire barrier be installed.
The licensee issued Nonconformance Report (NCR)
number 84-2144 to document the deficiency and establish corrective action.
The licensee has issued a
new FCR-E 3780 which clarifies the separation requirements.
The FCR was approved on January 9, 1985; however, the FCR still allows Non-Class IE conduit to be separated from Class IE open tray with only one-inch separation based on an analysis prepared by EBASCO design engineering.
This analysis is allowed by Section 5.1.1.2 of IEEE 384-1974 and Regulatory Guide 1.75.
The licensee gave the inspector a
draft copy of the analysis and stated that an approved copy will be available on April 1, 1985.
This analysis for reduced separation will be submitted to NRR for evaluation.
The item will be'n unresolved item pending NRR evaluation and FSAR updating.
This separation item will be identified as 400/85-08-02, Evaluation of Reduced Separation Between Non-Class IE Conduit and Open Class IE Raceway.
The deficiencies identified by the CAT inspectors and Region II inspector are a violation in that the design control has not been maintained.
The licensee has failed to assure that the applicable regulatory requirements for separation of Class IE and Non-Class IE electrical raceway components wer e correctly translated into specifications, drawings and procedures.
This viola-tion is identifed as 400/85-08-01,
"Inadequate Design Control for Raceway Separation".

Separation Deficiencies Between Redundant Class IE Wiring and Raceways Resolution The inspector examined the following raceways identified in Table II-2 by the CAT inspectors.
All deficiencies were verified.

C1808-SB$
L1801-SB 16229X-SA The above listings identify deficiencies where the required separation between redundant Class IE divisional wiring and raceways was not maintained.
There are two issues involved in this item:
(1) physical separation of the raceway and (2) the separation and protection of exposed cable from raceway to conduit.
The physical separation of raceway, item ( 1) will be corrected by the resolution of NCR-84-2144 and FCR-E-3780.
The routing of exposed cable is a result of the Harris design concept which allows cable to be run without physical protection and/or separation from the raceway to the equipment or conduits.
The licensee has issued FCR-E-3693 which adds specific requirements for the separation of cables between raceways and equipment.
The corrective actions to the presently installed cables will be resolved in accor-dance with the disposition of NCR 84-2183.
The above deficiencies are another example of the failure to control design activities as identified in paragraph 5(a).
Based on the above, Unresolved Item 50-400/84-44-02, which was assigned for the CAT finding in this area, is close M
