The inspectors also held discussions with other licensee and contractor personnel involved with Inservice Inspection activities.

Inservice Ins ection - Review of Procedures The Inservic'e Inspection (ISI) program plan and procedures were previously reviewed by the NRC as addressed in NRC Report No.
50-397/86-17.
The following procedures reviewed by the inspector represent revisions or newly implemented procedures to the ISI program:
gCI 6.2, Revision 2 of 3/23/87 "Examination of Piping Welds for IGSCC."
This procedure requires NDE personnel performing these examinations to be certified Level II or III UT examiners.
In addition, they must have been successfully qualified within the BWORG/NRC/EPRI coordination plan for detection and sizing of Intergranular Stress Corrosion Cracking (IGSCC) in BWR plants.
These special qualifications were on file and reviewed by the inspectors as discussed in Section 3 of this report.
This procedure requires all indications determined to be the result of IGSCC to be marked as
"REJECT" in the evaluation column of the Examination Data Sheet.
Inspector interviews with ISI personnel combined with reviews of the examination results revealed that, to date, no evidence of IGSCC have been identified.

The above procedures adequately addressed all areas specified in the licensee's ISI program.
No violations or deviations were identified.
Inservice Ins ection - Observation of Work and Work Activities The inspector observed Ultrasonic (UT) examinations performed on residual heat removal heat exchanger welds, numbers AC-1 (flange to shell circumference weld) and AN-3 (inlet nozzle to shell weld),
shown on ISI Drawing No.
RHR-214 Revision 1.
The following UT attributes were evaluated and found to be consistent with the approved procedure and ASME Section XI requirements:
The type of apparatus used, scanning technique, extent of coverage, calibration of the instrumentation and system prior to examination, beam angles, size and frequency of the search unit, limits of evaluation and recording of indications, and determination of acceptance limits.
In conjunction with this examination, the inspector reviewed the calibration block certifications, transducer RF pulse wave forms, and the ultrasonic couplant certifications.
The inspector also observed liquid penetrant (PT) examinations of RHR piping welds, numbers
RHR (1) A-14 (pipe to elbow) and
RHR (1) A-15 (elbow to pipe),
shown on ISI Drawing No.
RHR-201-2.
During the PT examinations, surface preparation, application of penetrant and developer (including recommended dwell times), penetrant removal, and visual inspection for surface indications were observed and found to be in compliance with the applicable procedure.
The qualification and certification records of the inspection personnel (Levels I, II and III) performing ISI examinations were reviewed by the inspector.
All personnel appeared qualified in accordance with ASME Section XI requirements.
A number of licensee and contractor personnel were also qualified for examination of piping welds for intergranular stress corrosion cracking (IGSCC), within the BWR Owner's Group (BWROG)/NRC/Emergency Plan Implementing Procedures (EPIP's) coordination plan for detection and sizing of IGSCC in BWR plants.
No violations or deviations were identified.
Inservice Ins ection - Data Review and Evaluation The inspector reviewed all the available NDE data sheets generated during this outage, that were reviewed and approved by the licensee's Level III and the Authorized Nuclear Inservice Inspector (ANII).
The records reviewed covered examination results and data sheets, examination equipment data, extent of examination, calibration data sheets, names of examiners, and examination evaluation data.
The data sheets reviewed by the inspector were for recording ISI examinations performed on items such as pipe welds, reactor pressure vessel welds, bolts, and welded lugs.
ISI methods included UT, PT, MT and visual examinations.
No adverse findings were reported on any of the ISI records reviewed.
No violations or deviations were identifie.
Inservice Ins ection (ISI Licensee Re ort The licensee submitted their Inservice Inspection Summary Report for Refueling Outage RF86A, to NRC Region V on August 22, 1986.
This report included the NIS-1 Owner's Data Report for Inservice Inspections for the Spring 1986 Refueling Outage and NIS-2 Owner's Reports of Repairs or Replacement completed since the filing of the Preservice Inspection Summary Report.
A sample of the ISI examination data was previously reviewed by the inspector and documented in NRC Report No. 50-397/86-17.
The ISI information provided in this report meets the reporting requirements of ASME Section XI, Article IMA-6000.
No violations or'eviations were identified.
Inspector discussions with gA personnel revealed that gA performs survei llances on ISI activities on a continuing basis throughout the outage.
Specific activities covered to date include:
review of NDE personnel certifications, review of calibrated test equipment used by the contractor (LMT) and the Supply System, and observation of PT and UT examination of pipe welds.
These surveillances were documented on a
draft report reviewed by the inspector; a final gA report will be issued at completion of the outage.
No violations or deviations were identified.
The inspectors met with licensee management representatives denoted in paragraph 1 on May 8, 1987.
The scope of the inspection and the inspectors'indings as noted in this report were discussed.
