LER 87-12.
-This LER reported an event involving a gaseous effluent release from the Turbine Building that was not monitored as required by the technical specifications.
The licensee identified the problem as a lack of procedural guidance to recognize indications of a sample flow ratio mismatch in the Turbine Building Exhaust Air Monitoring System.
As a corrective action Plant Procedure 7. 1.2, "Air Monitoring Sample Flow Control Ranges" had been revised to provide guidance in recognizing such flow mismatches.
The inspectors verified that the procedure has been modified to indicate acceptable flow conditions and actions to take when flow conditions are unacceptable.

This LER reported an event involving the ventilation exhaust from the Radwaste Building while its radiation monitor was out of service.
The licensee indicated that their initial corrective, action did not adequately address the problem and they plan to review the issue further to develop a more permanent resolution.
This item will remain open pending completion of licensee action.

Enforcement Item 87-24-01.
This item concerned a fai lure to properly label a container of licensed radioactive material.
As a corrective action the licensee changed the labeling of radioactive material containers so that the labels are covered with plastic and affixed to the containers with duct tape to prevent their loss due to weathering.

Failure to properly post radiation area in valve gallery area of the Radwaste Building.

Enforcement Item 87-14-03.
Failure to properly post a high radiation area at the recirculation pump work tent in the Radwaste Building.
In response to this item and item 87-14-02, above, the licensee has reevaluated their entire radiological posting program, and, as a result, the following improvements have been implemented:
Pad eyes have been installed to replace duct tape to secure postings and barriers.
38 x 46 inch metal signs suspended by chain in required areas read:
"CAUTION...TRANSIENT HIGH RADIATION AREAS IN THE OVERHEAD...NO ENTRY WITHOUT HEALTH PHYSICS APPROVAL".
The signs size and magenta color provide a conspicuous permanent warning to personnel.
Barrier swing gates have been redesigned to reduce.their tendency to stick in the open position, improve the stability of the gate base, and improve the return spring retaining hook.
Based on these actions, items 87-14-02 and 87-14-03 are closed.
Unresolved Items (Open) Unresolved Item 85-20-04.
This item was described in NRC Inspection Report 50-397/87-05, Section 2.9, and concerned plateout factors applicable to post Loss of Coolant Accident (LOCA) iodine sampling below 50 F in accordance with NUREG-0737, Item II.F. 1.2.
The licensee initially proposed to address this issue by establishing a jointly funded project in cooperation with several utility companies to contract with Science Applications International Corporation (SAIC) to study the problem.
The joint project failed to materialize and the licensee is pursuing an independent contract with the consulting firm.
This item will remain open pending results of licensee's action Unresolved Item An unresolved item is one about which more information is required in order to ascertain whether it is an acceptable item, a violation, or a deviation.
One unresolved item is discussed in Section 2 d of this report.
Radiation Protection Plant Chemistr and Radwaste:
Or anszatson and Mana ement Controls 83 22 This area was inspected to determine whether the licensee had maintained an organization that was adequately staffed and motivated to effectively control radiation, radioactive material, and plant chemistry.
The inspectors reviewed the current organization structure, and interviewed the Health Physics/Chemistry Manager and members of his supervisory and technician staff.
It was determined that the only significant changes in the organization since the last inspection was the addition of four health physics technicians to replace personnel lost through attrition.
All of the added personnel were qualified in accordance with ANSI/ANS 3. 1 1981 ("Selection, gualification, and Training of Personnel for Nuclear Power Plants" ) standards.
The licensee's Technical Specification 6.5.2.8 requires that an annual audit be conducted of the performance, tr aining, and qualifications of their entire unit staff.
The most recent audit performed in compliance with this requirement was guality Assurance (gA) Audit No.97-416, dated October 9,
1987.
The audit was performed by individuals having experience and training commensurate with the scope and complexity of the subject area that was audited.
The inspectors reviewed this audit and found it to be adequate in scope and depth to meet the intent of the requirement.
No significant findings were identified as a result of this audit.
By interviews with the Health Physics/Chemistry Manager and review of Radiological Occurrence Reports (RORs), the inspectors concluded that the licensee maintains effective control of radiation, radioactive material, and plant chemistry.
No violations or deviations were identified.
Trainin and ualifications:
General Em lo ee Trainin Radiation Safet Plant Chemistr Radwaste and Trans ortation 83722 This area was inspected to evaluate the effectiveness of'he training and retraining programs for non-licensed plant staff, contractors, and visitors who have responsibilities in the areas of radiation safety, plant chemistry, radwaste, and transportation.
Licensee Event Reports (LERs) and RORs issued since the last inspection were reviewed to determine if there were any indications of training deficiencies.
No such indications were noted by the inspector The licensee's General Employee Training (GET), both content and presentation, was reviewed by an NRC inspector during an inspection conducted in March 1987 and reported in Section 3 of NRC Inspection Report 50-397/87-05, dated March 31, 1987.
The inspector determined that the training was consistent with 10 CFR Part 19. 12, "Instructions to Workers."
No violations or deviations were identified in this program area.
Daily tours of the Turbine, Radwaste and Reactor Buildings were conducted during the inspection.
Members of the licensee's staff accompanied the inspectors on some occasions.
Independent radiation measurements were performed by the inspector with a Keithly, Model 36100 X-Ray/Gamma survey meter, Serial Number 10108, due for calibration on March 9, 1988.
The following observations were made:
o Work practices observed were consistent with instructions provided on Radiation Work Permits and the ALARA concept as defined in 10 CFR Part 20. 1(c).
o Portable instruments used for air sampling and radiation detection were in current calibration.
o Posted notices to workers were consistent with the requirements of
CFR Part 19.11.
o No unmonitored personnel were observed in the areas toured.
o In general, tools and equipment were properly stored.
o Posting and labeling practices were consistent with the requirements of 10 CFR Part 20.203.
o Housekeeping in the areas toured was much improved over that observed during previous inspections.
No violations or deviations were identified in this program area.
7.
Facilities and E ui ment 83727 E
This area was inspected to determine whether modifications or changes to the existing facilities and equipment for radiation protection activities had adversely affected the radiation protection program.
In discussions with the Health Physics/Chemistry Manager and by direct observation, it was determined that there had been no significant changes or additions to facilities or equipment since the last inspection.
No violations or deviations were identified in this program are.
Exit Interview The inspectors met with the licensee's representatives:at the conclusion of the inspection on November 20, 1987 and summarized the scope and findings of the inspection.

The licensee was advised that no violations or deviations were identified as a result of this inspection.
