Areas Ins ected:

Unannounced inspection by a regional inspector of the implementation of the WNP-2 Nuclear Power Plant Fire Protection Program, Quality Assurance Program, and other items identified during the outage.

During this inspection, Inspection Procedures 30703, 62700, 90712, 64704B and 92701 were covered.

Results:

Of the areas examine'd violations were identified in the fire protection area (see paragraph 4).

IN-86-47 (Closed3 Erratic Behavior of Static "0" Rin Switches The licensee's organization had been working with the La Salle licensee's staff and had full knowledge of the reactor trip problem and possible cause due to static "0" ring level switch malfunctions.

This item will be followed up in future inspections as IE Bulletin 86-02.

Maintenance (IE Manual Cha ter 62700)

The inspector reviewed twenty corrective and preventive maintenance procedures and determined that:

The required administrative approvals were obtained LCOs, where applicable, were met Approved procedures were used Inspections were in accordance with licensee requirements Testing was completed prior to return to service QC records were complete Records were assembled and stored as part of maintenance history The licensee has a large number of MMR's outstanding and is currently taking action to reduce the backlog.

In addition, the licensee has a

system for prioritizing the items so that safety related or important-to-safety MWRs are not neglected.

service requirements, technical specification compliance and radiological hazards were properly considered.

Of the areas examined no violations or deviations were identified.

Plant Tour During the plant tour which the inspector conducted, the following observations were made.

Plant equipment material condition appeared good.

No radiological controls problems were observed.

Plant housekeeping was poor.

Violations in inspection reports 85-05, 85-20 and 85-22 identified discrepancies in the control of combustible materials.

Inspection report 86-04, item 86-04-01, discussed housekeeping problems in low traffic areas of the plant.

Internal licensee memoranda dated 4/30/86, 5/8/86, 5/16/86 and 6/9/86 identified major, problems in this area.

The inspector concluded,,while in th'e facility on 6/12/86 'after normal plant working hours, with,the reactor operating at power, that at quitting time plant personnel had apparently ma'de very little effort to properly store the materials that they"'had Seen using to do work..'he following discrepancies were observed by the inspector at various times during the inspection and did'not meet NRC fire protection requirements and the licensee's procedural requirements, specified in PPH 1.3.35.

On June 1'l", 1986 the:inspector obs'erved after working hours:

, A 1 pint plastic spray, bottle (not an approved safety container)

labeled "alcohol" on the '606'levation. of the Reactor Building.

The bottle was still there on June 12, 1986.

Fire door R 5-17'on the 522'levation of the Reactor Building propped open with,no fire systems impairment permit.

This door was also propped open on 6/12/86.

2 large cardboard boxes with combustibles unattended on the 522'levation of the Reactor Building.

These were still there on 6/12/86.

No transient combustible permit was in the area.

A large cardboard box on a wood (apparently untreated)

block stand beside the SLC area on the 501'levation of the Reactor Building.

These materials were also observed on 6/12/86.

No transient combustible permit was in the area.

b.

On June 12, 1986 a.m.~ while with licensee personnel, the inspector also observed:

Fire door R-408 to a motor control center on the 522'levation of the Reactor Building propped open with a wooden broom with tape over the latch to prevent latching and also rendering the

door inoperable.

No fire system impairment permit was observed.

On 6/12/86 after working hours the inspector observed:

Xn the Reactor Building:

On the 606'levation, in a neat pile, two brag boxes of paper towels and various other cleaning materials.

No transient combustible permit was observed.

On the 522'levation the inspector observed:

A cardboard box full of plastic sheets in front of the electronics counting room.

No transient combustible permit was observed.

Two small piles of cloth and plastic, one under IR 71 and one under RRC PT 38A.

No transient combustible permit was observed.

A 10 gallon trash can so full of oily paper that it was propped open (also observed on 6/ll but not emptied).

The label on the can said to empty nightly.

No transient combustible, permit was observed.

A pile of trash on the floor including cloth, plastic and cardboard in a corner under TB&R 494.

These items were also observed on 6/ll/86.

No transient combustible permit was observed.

In front of the primary containment entry (also observed on 6/11'/86)

was an accumulation of combustible materials both xnside and,,outside of a'oped off area.

On the 548'levation the inspector observed:

No transient combustible permit was observed.

A large plastic cart filled with full bags of radwaste and PC's left unattended in the S.E.

corner.

'o transient combustible permit was observed.

A plastic room built for thermolag work containing

cardboard boxes and 1 brag box.

Outside of this room was a large cardboard box of filters.

No transient, combustible permit was observed.

On the 501'levation the inspector observed:

In the NM corner a pile of hoses and a brag box of

'paper towels, no transient combustible permit was observed.

Inside of the shielded area in the NW corner, a very large yellow poly bag with some clothing in it, loose in.a,pile in the area (observed 6/11 and 6/12).

No transient combustible permit was observed.

cardboard box,with plastic inside.

No transient combustible permit was observed.

In the Control Room Emergency Ventilation Room, a pile of hoses and electrical cord.

No transient combustible permit was observed.

In Division 1 Switchgear Room, Room C208, an aerosol can of enamel labeled "Extremely Flammable" stored in an open crew box and two rolls of cable on the floor.

No transient combustible permit, was observed.

In the Remote Shutdown Room a cardboard box of trash was observed on 6/11 and 6/12.

Mo transient combustible permit was observed.

Licensee procedure A.P. 1.3.35 lists various methods to control the use of combustible materials inside of plant areas and provides methods to control impairment of fire barriers.

These were apparently not being implemented effectively.

This violation is of a repetitive nature and makes it appear to the inspector that the corrective actions taken as a result of the previous violations have not been effective.

The licensee by letter dated May 28, 1986, committed to participate in the BWR Owners'roup (BWROG) program for resolution of this issue.

On 4/29/86 the BWROG submitted a letter to the NRC identifying that a task force had been formed to evaluate the issue.

On 6/3/86, J. Taylor (NRC) responded to the BWROG providing a date of 9/1/86 for completing generic work.

86-05-03 (Closed) Pire Protection Construction A

The inspector reviewed QA inspection procedures currently in use by the licensee for fire protection systems.

They appeared to be adequate.

c.

86-05-02 (0 en) Thermola Work The inspector observed work on the thermolag being installed during the time on site.

By telecon on 7/7/86 and 7/10/86 the inspector discussed with licensee personnel the completion schedule for this work.

The licensee's original planned completion date was July 31, 1986.

After completion of this inspection, the licensee was discussing a possible need to extend this schedule to mid-August 1986 due to the increased scope of work.
