On September 30, 2020, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Fermi Power Plant, Unit 2. On October 20, 2020, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Fermi Power Plant, Unit 2, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Unit 2 began the inspection period shut down for a planned refueling outage. Following completion of refueling and other maintenance activities, the reactor was restarted and taken critical on July 30, 2020. The main generator was synchronized to the power grid on August 5, 2020. The unit reached full power operation on August 12, 2020. On August 21, 2020, the unit was down powered to 68 percent rated thermal power for a planned control rod pattern adjustment. The unit was returned to rated thermal power the following day. The unit remained at or near rated thermal power for the remainder off the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Starting on March 20, 2020, in response to the National Emergency declared by the President of the United States on the public health risks of the coronavirus (COVID-19), resident inspectors were directed to begin telework and to remotely access licensee information using available technology. During this time the resident inspectors performed periodic site visits each week and during that time conducted plant status activities as described in IMC 2515, Appendix D; observed risk significant activities, and completed on-site portions of IPs. In addition, resident and regional baseline inspections were evaluated to determine if all or a portion of the objectives and requirements stated in the IP could be performed remotely. If the inspections could be performed remotely, they were conducted per the applicable IP. In some cases, portions of an IP were completed remotely and on-site. The inspections documented below met the objectives and requirements for completion of the IP.

71153 - Followup of Events and Notices of Enforcement Discretion Event Followup (IP Section 03.01)

(1) The inspectors evaluated the licensee's immediate actions following a Division 2 loss of offsite power and Division 2 emergency diesel generator 13 and 14 automatic actuation as a result of a mayfly infestation in the 345-kilovolt switchyard (NRC event notification 54761) during the week ending July 4, 2020

Event Report (IP Section 03.02) (1 Sample)

The inspectors evaluated the following licensee event reports (LERs):

(1) LER 2020-001: Secondary Containment Inoperable Due to Both Airlock Doors Open (ADAMS Accession No. ML20114E190). The inspectors determined that the cause of the condition described in the LER was not reasonably within the licensees ability to foresee and correct and therefore was not reasonably preventable. No performance deficiency nor violation of NRC requirements was identified. Secondary containment was restored within the Technical Specification required timeframe and secondary containment pressure remained with the Technical Specification operability limits.

INSPECTION RESULTS

Observation: Adverse Configuration Control Trend 71152 Controlling plant configuration of SSCs is an important aspect to maintaining plant design functions and defense-in-depth (e.g., multiple trains or divisions of SSCs or multiple SSCs to perform the same design function). A mispositioned SSC has the potential to adversely impact plant equipment resulting in problems that could cause initiating events or impact the availability and functional capability of mitigating SSCs or barriers. During calendar year 2020, the licensee identified a continuation of an adverse trend of configuration control issues, often referred to as plant status control or component mispositioning. The licensee entered the trend into their corrective action program along with the various individual configuration control issues. Some configuration control event examples included, but were not limited to:

  • On January 20, 2020, a licensed operator placed drywell cooling fan #2 CMC switch in run vice auto during a planned dedicated shutdown panel surveillance test. A peer check (informal concurrent verification) by another licensed operator did not ensure the procedure was followed. There were no adverse plant impacts as the drywell cooling fan was being manipulated off and on during a planned surveillance test and was turned on as desired but with the switch not in the intended position. [CARD 20-20646]
  • On February 27, 2020, a non-licensed operator re-installed the electric fire pump contactor in the closed position (vice open) to restore the system from planned maintenance and the electric fire pump automatically started. The procedures for system restoration did not provide specific guidance to ensure the contactor was open prior to installation. There were no adverse impacts to the plant as the electric fire pump had not yet been declared functional and an auto-start of the electric fire pump does not impair the ability of the fire protection system to perform its intended function.   • On March 26, 2020, a non-licensed operator opened a 120 volt alternating current (VAC) circuit breaker in the incorrect panel - H11P926 instead of H11P927 (both 120 VAC balance of plant distribution panels for the integrated plant computer system

[IPCS]) - while performing a clearance and tagging evolution to provide worker protection for the IPCS replacement project. The operators in the field did not perform a concurrent verification as discussed during the pre-job brief. There were no adverse plant impacts as the plant was in hot shutdown (Mode 3) and the plant computer was planned to be removed from service for planned replacement. [CARD 20-23288]

  • On May 29, 2020, instrumentation and control technicians installed a jumper in the incorrect auxiliary trip unit (D11K607B for off-gas radiation monitor 'B' instead of D11K607C for radwaste effluent radiation monitor) during planned radwaste effluent monitor functional testing. The result was the off-gas radiation monitor 'B' was rendered non-functional instead of the radwaste effluent monitor. The technicians did not perform a proper concurrent verification to ensure they were on the correct component. There were no adverse plant impacts as the off-gas system was out-of-service and the off-gas radiation monitor was not required while the plant was defueled (no mode). [CARD 20-26280]
  • On July 28, 2020, instrumentation and control technicians lifted an electrical lead and installed two jumpers in the incorrect panel (Panel H11P613 (Division 1) instead of Panel H11P612 (Division 2)) in the relay room during Division 2 anticipated transient without scram and safety relief valve low-low set reactor vessel pressure functional surveillance testing. The technicians did not perform a concurrent verification as required to ensure they were on the correct component. There were no adverse impacts on plant operation as the plant was in cold shutdown (Mode 4) and outside the Modes of applicability for the associated technical specifications for the specified components. [CARD 20-28744]
  • On August 7, 2020, the main turbine lube oil upper heat exchanger inlet valve was discovered open allowing bypass flow around the lower heat exchanger that was in-service. The main turbine lube oil system valve line up procedure did not contain sufficient guidance to ensure the associated general service water system valves were in the correct position. As a result, while operating at approximately 35 percent rated thermal power, the turbine lube oil temperature control valve controller indicated full open with turbine lube oil temperatures higher than expected given plant conditions at reduced power. There were no adverse impacts on main turbine operation as the lube oil temperature did not exceed procedural limits.

The inspectors noted several additional examples that were not formally classified as plant status control events per licensee procedure MOP 05, Control of Equipment, (did not meet threshold for various reasons) but indicated continued challenges with configuration control.

Examples included but were not limited to:

  • On April 15, 2020, the breaker switch for the Division 2 battery room east exhaust fan was bumped from the on position to the off position while contract electricians were attempting to open the door to the associated motor control center to perform component phase rotation checks. This resulted in the fan being de-energized.

There were no adverse impacts to plant operation as the system was under maintenance and not required while the plant was defueled. [CARD 20-24552]

  • On April 25, 2020, a shorting link was discovered installed across the incorrect terminal block terminals on a 24-volt direct current power supply for the replacement IPCS. The issue was believed to have occurred during reassembly and installation of the terminal blocks during refueling outage 20 (began March 21, 2020). There were no adverse impacts to plant operation as IPCS was out-of-service for planned replacement while the plant was defueled. [CARD 20-25183]
  • On April 30, 2020, a temporary HVAC vent valve was discovered in the open position that partially drained chill water from an in-service cooler in support of temporary torus area ventilation. The component was believed to have been bumped. There were no adverse impacts to plant operation as there were multiple temporary HVAC systems for the torus which was out of service for planned maintenance while the plant was defueled. [CARD 20-25470]

The inspectors noted most configuration control issues involved human performance errors in not following a procedure or process. Not performing concurrent verifications properly was a common contributor. These contributors were similar to those associated with the previously identified adverse configuration control trend documented in NRC Integrated Inspection Report 2019004 (ADAMS Accession: ML20038A340). The previous corrective actions included additional configuration control questions embedded into pre-job briefings. At the end of this inspection period, a common cause evaluation of the various configuration control events was in-progress as a corrective action to address the adverse trend.

The inspectors did not identify any violations of regulatory requirements or performance deficiencies of more than minor significance based on the circumstances stated above.
