Meetina Summary Opening remarks made by NRC management summarized NRC concerns regarding repetitive losses of RHR Flow, Physical Security Plan implementation, Health Physics Appraisal results, and ongoing licensee corrective actions on Systematic Appraisal of L1censee Performance (SALP) findinas.

a.

Repeti ive Losses of Residual Heat Removal System (RHR) Flow NRC concerns related to procedural adequacy and the licensee's feedback system to factor in operational experience and prior history to assure adequacy of corrective action were discussed.

The licensee is imple-menting procedural changes to provide further assurance that recurrence of similar events will be minimized.

Management cortrols for evaluation, correction and prevention of future incidents was addressed.

The licensee was also apprised of 10 CFR 50.72 reporting requirements with respect to the March 5,1981 event, which impaired RHR capability, when an erroneous remote level indication resulted in an inadvertent decrease in actual Reactor Coolant System (RCS) water level (Reference:

Immediate Action Letter - IAL 81-14, dated March 9,1981).

Physical Security Plan Implementation An NRC assessment of security equipment and program status was provided to the licensee.

Physical security enforcement history, contractor security force training, and key card usage was discussed.

The licensee is now assessing various methods to provide additional

supervision for the contractor security force.

c.

Health Physics Appraisal Results An NRC overview of the recent NRC Health Physics Appraisal was provided to the licensee.

Discussions were held regarding cali-bration facilities, adequacy of documentation for calibration of survey meters (source strength), maintenance of instruments, beta monitoring, respiratory protection, air sampling, liquid waste

management, work space constraints, and housekeeping.

Status of Licensee Systematic Appraisal of Licensee Performance (SALP) Action Discussions were held addressing prior NRC concerns regarding maintenance (I&C), plant operations, QA/qC committee activities, and management controls.

The licensee's planned reorganization was discussed in detail including planned staffing increases.

The licensee plans to create a new Nuclear Division within the

,

Duquesne Light Company structure.

The Nuclear Division Head will report directly to the President of Duquesne Light Company.

Approval of the new organization was projected to be on or about i

March 24, 1981, after presentation to the Board of Directors.

No immediate impact on operations staff was expected.

The time frame for complete implementation of the licensee's proposed reorganization was undefined as of the date of this meeting.

Commitments The licensee was requested to and committed to the following:

Notify Region I upon approval of the new organization structure with a written report by April 3,1981 addressing the reor-

ganization plan including Technical Specification submittal plans.

The licensee was advised that periodic status reports with respect to hiring of experienced engineers, other technical personnel and the areas assigned should be provided to the resident inspectors.

Further, detailed organizational charts and milestones should be made available to the Resident Office.

Report to Region I by April 3,1981 of the status of plans for implementation of management controls to address specific program-matic, procedural and/or equipment deficiencies.

The licensee was advised that controls should address actions taken to ensure that operating, surveillance, and maintenance problems which can effect the safety of operations are formally identified to senior station or corporate management.

Discussions were held to assure that plans and schedules would be provided to the NRC identifying those tasks which will be done during the transition to the new organization and after its implementation in those areas in which deficiencies have been identified.
