held by Mr. Beckman with Messrs. H. Williams and J. Sieber of your staff at the conclusion of the inspection.

Areas examinad during this inspection are described in the Office t f Inspection and Enforcement Inspution Report which is enclosed vita this letter. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, measure-ments made by the inspt:ctor, and observation by the inspector.

Our inspector also verifled the stss you nave taken to correct the items or noncomp1 6 ce brought to your e t% tion in letters dated May 19, June 17 and

June 25, !M1. We have no furtw cuestions regarding the steps you took to correct items A and F of the Hay 19 letter, Appendix A; item A of the June 17
letter, and items C and D of the June 25 letter.
Based on the results of this inspection, it appears that certain of your i
activities were not conducted in full compliance with NRC requirements, as set forth in the Notice of Violation, enclosed herewith as Appenidix A.
These items of noncompliance have been citegorized into the levels described in the Federal Register Notice (45 FR 66754) dated October 7,1980.

Appendix B Item A in the Uotice of Violation enclosed with this letter was

icentified during a previous inspection of your licensed activities on February 1-28, 1981 and was documented in the enclosure to our letter dated

June 17,1981. Your letter to this office dated July 10, 1981, stated the
!
cause of this violation was personnel error; disciplinary action was taken
!
for involved individuals; and, that action was expected to deter recurrence.
!
From our July 6 - August 3,1981 inspection it appears that the stated correctiva actions were not effective since this item has recurred.
Recurrent and uncorrected items of noncompliance ere ghen additional weight in the consideration and selection of appropriate enforcement action. There-fore, in your response to this letter, you should given particular attention to thole actions taken or planvd to ensure that identified items of noncom-pliance w ll be completely corn.cted and will not recur.
i In addition to the recurrent item discussed above, Appendix A, Items A and B are similar in nature to violations identified in IE Inspection Report "los.
50-334/80-11 and 80-09, and 30-27, involving failures to s6tisfy Technical Specification mquirements as prerequisites for rr. fueling or startup activities.
In general, these violations involved omissions or ambiguities in facility procedures or checklists. Accordingly, your response to items A and 8 of l
Appendix A should address those aci, ions which you have taken to ensure that additional omissions or ambiguities in existing prerequisite checklists, l
Technical Specification requinsmnts, and/or surveillance tests are identified and corrected.
In accordance with Section 2.790(d) of the NRC's " Rules of Practice," Part 2, l
Title 10, Code of Federal Regu'ations, documente. tion of findings of your l
contml and accounting procedures for safeguarding spectal nuclear materials and your facility security measures for physical protection are deemed to be concercial or financial information within the meaning cf 10 CFR 9.5(a)(4) and i
shall be subject to disclosure only in accordance with the provisions of 10 CFR 9.12; therefore, the pertinent section of the enclosed inspection report and Apendix B, will not be placed in the Public Document Room and wili receive limited distribution.
