Inspection on November 18, 1981 by a representative of the State of Washington Department of Social and Health Services The Power Authority of the State of New York was the originator of a waste shipment which was. inspected at the U.S. Ecology, Inc. burial site at Richland, Washington.
Results: One apparent item of noncompliance was identified (delivery -
of radioactive material to a carrier for transport in a container that was not a strong, tight package).

s DETAILS On November 13, 1981 the Power Authority of the State of New York dispatched a shipment of licensed material consisting of eight boxes of LSA radioactive waste,- containing 971.6 millicuries, to the U. S. Ecology, Inc. _ burial site at Richland, Washington.
The shipment was made in an exclusive use open vehicle.
On November 18, 1981, a representative of the State of Washington inspected the shipment at the burial site. The inspection consisted of a review of shipping papers and placarding, radiation measurements, selective contamination surveys, and examination of the packages and the tractor-trailer.
A copy of the' report prepared by the representative is attached. The report states that upon inspection one of the boxes (Box No. 74-81 containing about 10 mil 11 curies of radioactive waste) was found to be leaking a liquid which appeared to be oil.
In a December 15, 1981 telephone conversation with P.
Clemons, a State of Washington representative reported that at the time of the inspection, liquid was leaking from all four corners of the box.
10 CFR 71.5 prohibits delivery of licensed material to a carrier for transport unless the licensee complies with applicable regulations of the Department of Transportation in 49 CFR Parts 170-189.
49 CFR 173.392(c)(1) states, " Materials must be packaged in strong, tight packages so that there will be no leakage of radioactive material under conditions normally incident to transportation." -
Box No. 74-81 was not a strong, tight package in that liquid was found to be leaking from it on November 18, 1981. This represents noncompliance with 10 CFR 71.5 (81-29-01).
The James A. FitzPatrick Assistant to the Resident Manager was ir. formed of this apparent item of noncompliance in a telephone conversation with P. Clemons on December 24, 1981.
