On May.8,-1989, NRC Region I issued the Augmented Inspection Team's-(AIT) eval-uation and conclusion .of'the Reactor Core Isolation Cooling (RCIC) overpress-urization event .(Inspection Report 50-293/89-80). The Team concluded that the safety significance of the April 12, 1989,- pressurization transient was minor -

since - several barriers remained intact or -available to mitigate. the effects of

ENCLOSURE Pilgrim Status Report for the Period May 6-19, 1989 l

1.0 Plant Status As of 8:00 a.m. on May'19, 1989, the reactor is in the cold shutdown mode with moderator. temperature about 117 degrees .0 Power Ascension Test Program Status The licensee has completed the original scope of the Power Ascension Test Program up to the 25% power plateau. Prior to continuation of the power ascension program to the 50% power platcau, the licensee must obtain the NRC Region 1 Regional Administrator's concurrence. The program includes NRC approval points at 5%, 25%, 50% and 75% of full power and both a licensee written report and NRC review after completion of testing at full power. The NRC Restart Assessment Panel will recommend to the Regional Administrator when the plant is considered ready to be released from each approval poin .0 Facilities Operations Summary As reported in the last Pilgrim Status Report (April 22 - May 5,1989),

the plant was in cold shutdown at the beginning of this report period following the turbine trip and resultant reactor scram on high reactor vessel water level. The licensee concluded that malfunction of a Feed-water Regulating Valve (FRV) caused the momentary water level increase in the reactor, which lead to the turbine generator trip and reactor shut-down. The licensee attributed the spurious closures of five containment isolation valves to a voltage transient in the power supply to the valves during a fast transfer of power sources upon the turbine generator tri Detailed NRC review of the above event and licensee actions will be docu-mented in Inspection Report No. 50-293/89-0 The plant remained in the cold shutdown mode throughout this report period while the licensee completed troubleshooting of the causes that lead to the May 3,1989, reactor scram and other maintenance items started during this outage. Enclosure 2 l ~4.0' Items of Special Interest Licensee Personnel Reassignments On May 10,1989, the licensee announced that Mr. Jack Alexander assumed the responsibilities of the Training Department Manager and Mr. Edward Kraft assumed the responsibilities of Deputy Plant Manager, a newly created position. Mr. Alexander was the former Operations Depart-ment Manager and 'Mr. Kraft was the former Training Department Manage The licensee also announced that their Regulatory Affairs Manager has accepted a position with another firm beginning June 1989. Mr. Vern 0heim, currently the Nuclear Engineering Department Deputy Manager has been assigned to act as Regulatory Affairs Manage .0 Emergency Notification System (ENS) Reports On May 18,1989, while licensee personnel were attempting to clear a rod block, a reactor scram occurred when the mode switch was taken from the refuel to the startup position while a high scram discharge volume condi-tion existed following replacement of one of the Intermediate Range Monitor bypass switches. During rod withdrawal, a rod block prevents excessive local power by blocking rod withdrawal after local power reaches preset limit The scram discharge volume receives and contains water discharged from all associated hydraulic control units during a scra Since the plant was in cold shutdown at the time, no rod movement occurre The licensee informed the NRC via EN Detailed NRC review of the above event and licensee actions will be documented in Inspection Report N '50-293/89-0 .

6.0 Pilgrim Restart Staff Activities The Pilgrim restart staff was in extended coverage at the beginning of this report period, consistent with reduced testing activity. The restart staff began around-the-clock shift coverage at 6:30 a.m. on May 18, 198 Additional personnel from NRC Region I, the Office of Nuclear Reactor Regulation (NRR) and an NRC contractor were onsite during the report period to augment the resident inspection staf l A NRC-developed inspection plan addressing the 5-25's power plateau and the primary functional areas of operations, maintenance, surveillance, and startup testing continued to be implemented.
