A management . meeting was held at. NRC Region I on March 9,1989, to discuss li.censee followup to the. March 4,1989 reactor scram. DETAILS 1.0 Purpose The management conference was held at the request of the licensee, Boston Edison Company (BECo). The licensee wanted to discuss ~ completed and planned followup actions in regard to -the March 4, 1989 reactor scra Meeting attendees are listed in Attachment .0 Meeting Summary-Mr. William Russell, NRC Region I Regional Administrator, opened the meet-ing with a brief introduction of the - topic and personnel in attendanc Mr. Ralph Bird, Boston Edison Senior.Vice President, presented an overview of Boston Edison's planned presentation and' the purpose of the meetin BECo distributed a Boston Edison prepared package of the intended presen-tation including the overhead displays (Attachment II).

The licensee then made a presentation on the March 4,1989 reactor scra The event involved. an unplanned isolation of the main steam lines and attendant reactor scram when pressure dropped as a result of spurious operation of the nonsafety related turbine bypass valve All safety systems functioned properly and there were no adverse safety consequences-of the even ]

The licensee provided schematics and drawings of the turbine control sys- -

tem and explained system operation. The reactor scram of March 4,1989, was then discussed in detai The events leading to the scram and the events immediately following the scram were analyzed using information i from the process computer, the Emergency Plant Information Computer (EPIC), and various instrument recorder strip charts. Through interviews with watchstanders and observers the licensee identified the location of all people in the control room at the time of the even The . licensee performed visual inspections and a functional test of affec- I ted systems and components as part of their' followup actions. They also conducted a review of instrument data and investigated the possibility of similar industry events. An independent review of the event was also performed by the engineering department. At the conclusion of the inves-tigation, the licensee identified several potential causes, but could not conclusively determine that any of these was the actual cause of the event.

s The licensee proposed a test to be run .after startup to confirm proper operation of turbine controls or identify the cause of. the March 4,1989 event. This test would include all the conditions leading to that even In addition, a ' closed circuit TV would be installed to monitor the link-ages at the turbine statio Additional instrumentation' would also be utilized. The licensee would use the test results to determine what, if any, additional actions are require .0 Concluding Statements At the end of the meeting, Mr. Russell and Mr. Bird made concluding remarks. Mr. Russell questioned if the as built conditions are in accord-ance with the drawings. The licensee responded that a large portion of this verification -work had been completed, but they were evaluating it further at the time. Mr. Russell also asked if there was a circuit-to-ground or circuit-to-circuit interaction. The licensee stated that they

- have performed that type of testing and will review that testing to deter-i mine if it is complete. (Subsequently, the licensee provided acceptable answers to the above questions in a teleconference on March 11,1989.)

The licensee comraitted to a complete evaluation of the turbine test and to provide the NRC Restart Manager with the results prior to resuming with the Power Ascension Program.
