Company during operation of the Pilgrim Nuclear Power Station Power Ascension Program, including operation at full power. The results of this review,;along-l with the status of emergency preparedness for the comunities surrounding the facility, are shown in enclosures to the attached draft letter to Boston Edison, Please note that the staff intends to issue!s n or about February 9.- 1990, the o
- attached letter to: (1)acceptthelicensee completion and final assessment i
of the Power Ascension Program and (2) close out Confirmatory Action Letter 86-10. Accordingly, the staff would end its special NRC_ Restart Staff'inspec-i tion program and NRC Restart Assessment Panel oversight of Pilgrim.
- However, NRC attention to Pilgrim will remain' above that typical of ~a single unit ' site.
For example, a staff of three resident inspectors and enhanced technical and management attention will be devoted to the site until the staff has determined that NRC ovb sight can be reduced to that normally devoted to a single Unit site.
In order to keep the Comission apprised of off-site Emergency Preparedness I
(EP)' issues, the staff plans to provide status reports, initially on about a l
quarterly basis. The repor'- format will be similar to the EP status sumary provided herein. As in the past, the information will be gathered in coordination i
with the local communities.
This action has been coordinated with the Office of the General-Counsel, which 4
has no legal objection to this action.

This letter provides NRC acceptance of Bostoh Eatsc Company's (BEco) comple-i tion and self-assessment of the Power Ascension Test Program and satisfaction of NRC Confirmatory Action Letter 86-10 and its supplemer.?s.
This letter also serves to close Confirmatory Action Letter (CAL) 86-10 reterence(2). This-
-letter neither imposes any new requirements nor release,s Boston Edison from any existing requirements contained in the Facility Operating License or Technical Specifications. The staff finds that management performance, plant material condition and operational performance support proceeding with normal operation 3'
of the facility.
The bases for approving the completion of the Power Ascension Program and re-lease from CAL 86-10 consist of: (1) successful completion of the-Power Ascen-i sionProgran;(2)acomprehensiveassessmentofplantandpersonnelperformance by BEco during this phase of plant operation; (3) Final Assessment Report (FAR) submittal by BEco and review of the FAR by the NRC; and (4) HRC staff inspec-tion and review activities which have confirmed the effectiveness of. both the staff and management of BEco during~this phase of plant operation in assuring safe operation of the facility. Enclosure 1 is a surinary of NRC findings which either have been or will be documented in NRC inspection reports.-
However, NRC Staff identified the following three areas in particular from your FAR that are key to long-term improvement and will therefore place special.

December 8, 1989 Automatic reactor scram from about 955 power on a false low reactor water level signal during calibration of a reactor vessel level instrument.

December 11, 1989 The plant was returned to service. Power reached about 95% on December 14, 1989, and remained there because the electrical grid load dispatcher requested the plant to defer a power reduction to adjust the control rods to their full power pattern.

One equipment problem identified during the Power Ascension Test Program remains unresolved: the tripping of the "A" recirculation sump motor-generator (M G) while in manual-remote mode at hig1 flow conditions. On October 7, 1987 with reactor power at about 895, on October 12,1989 with rea nor at 100f and subsequently on December 6,1989 with reactor at about-t 971, the M-G set tripped when the generator field breaker opened due to generator field undervoltage.
Each time, reactor power stabilized at about 55% to 60% of rated power following the 1
transient. 1.icensee investigation and troubleshooting have failed to identify a conclusive reason for these M-G set triss.
The "A" Recirculation M-G set scoop tube is " locked up" at tiis time.
4 On January 11, 1990, one of the five deep draft salt service water pumps was dropped about one foot to the ground after re-

l moval from the system as it was being positioned for mainten-ance.
The drop caused brittle fracture of one of the pump's 1
five eight-foot shaft casing columns. Based on the design mate-l rial properties on the column it should have failed in a duc-i tile manner if at all. The IIcensee is conducting an expedited-evaluation o,f this event and also plans to inspect.the other four pumps once the fifth is~ repaired. The licensee is confi-dent that the stress of the drop was significantly higher than any combination of design stresses for an installed pump and is completing. confirmatory calculations. Msnagement/ personnel Performance and Oversight' Activities I
r The NRC Restart Staff has continued to monitor management and personnel performance since start-up. BEco management has been generally responsive to NRC concerns and has acted in a cautious, deliberate manner.
Licensee peer evaluators were observedandhavebeenprovidinggoodfeedbacktoplantper-sonnel. Operators were general y knowledgeable of their duties and plant conditions and management maintains an active and effective oversight of operations.
Operators responded very well to events that occurred during this phase of the PATP.
These events included three recircula-tion pump motor-generator trips and one reactor scram.
The Restart inspection Staff observed Phase !! of the licensee's i
ShutdownfromOutsidetheControlRoom(SDOCR) test. The licen-see's preparation, comunications, and comand and control were excellent. Test personnel were aware of their re,sponsibilities, skilled in their assil)nments,. and demonstrated excellent plant knowledge, quick thin ting and effective teamworke Frompreparationthroughexecution,thistestwasferformedin a safe and satisfactory manner. Phases'l and 11 o the Shutdown-from Outside Control Room Demonstration were two cases of exem-plary performances by the licensee's operations department dur-ing the PATP.
During the 75% to.100% power plateau, management performed in a-generally cautious deliberate manner. Management was observed maintaining an active oversight of operations and actively l

-6o involved in decision making.1.icensee evaluation and critique of events continue to be comprehensive and properly focused, demonstrating a safety conscious attitude.
From October 13 through November 6,1989 - the licensee conducted amini-outageinordertoperformschedu!edmaintenanceandsur-veillance activities. The NRC Restart Staff determined that overall the October mint-outage was conducted in an extremely professional,well-organizedandconservativemanner.
Particu-larly noteworthy were the conservative management decisions made at several junctures during the outage. Operations staff cogni-rance of maintenance activities during the outage was superior to that observed during the preceding nine months of the PATP.
During this assessment period, the NRC Restart $taff audited selected tagout documents generated during the October mainten-ante outage. The audit consisted of about fif ty completed tag-outs and five active tegouts.
Review of the tagouts revealed that over 80% of the tagouts contained some error or omission.
Over 30% of the reviewed tagouts had significant errors such as tags hung or cleared by individuals other than those specified on the tagout and, in one case, tags cleared without Nuclear Watch Engineer signature.
In a sampling of approximately ten tagouts involving work which required the lif ting of leads or the installation of jumpers over 50% of the jobs were not entered in the lifted Lead a,nd Jumper Log. Four of the in-stances noted involved the replacement of safety related com-ponents (e.g. Reactor Level Transmitter; Diesel Generator Emer-gencyStartRelay).
The licensee confirmed the NRC findings, identified possible causes and has detailed a corrective action and prevention pro-Included as part of this program is the promulgation of a gram.
new procedure requiring periodic tag audits and a one-time Qual-ity Assurance tagging audit to be completed early in 1990.
It should be noted that no hardware problems were identified (e.g.
tags were hung on the correct valves). Licensee follow up of this issue has been appropriate.
These findings confirm previous NRC assessments that have con-ciuded that, while performance is acceptable and continues to improve, the areas of. procedural adherence and attention-to-detail require continued licensee management attention and NRC monitoring.
The NRC Restart Assessment Panel has concluded that the licensee management and plant staff performance support continued full power operation.

The NRC panel has concluded that plant operations are being conducted safely and conservatively.
The NRC has reviewed BEco's actions in response to inspection findings which had potential to impact further reactor opera-tions and found no outstanding issues which would preclude-the

Performance of a Final PATP 5 elf-Assessment, and 5atisfaction of Confirmatory Action Letter (CAL) 8610 The Staff also performed a retrospective overall review and evalu-ation to assess whether the licensee has successfully and effectively completed the Power Ascension Test Program,-has performed a meaning-ful and critical self-assessment and is making appropriate short-term and long-term use of lessons learned.
In the course of this review the statf placed special emphasis on assessingt (1) the licensee's,
l plans and measures for transition from the Power Ascension Program to lessonslearnedIn(2)thelicensee'smeasuresforincorporating normal operation topermanentprograms,and(3)thelicensee'splans for continued self evaluation, self-assessment, and self-improvement.
During its deliberations, the Staff evaluated whether-the licensee had fully satisfied CAL 86-10 and whether the CAL should be closed out.
3.1 Background
The Second Supplement to Con'irinatory Action Letter'(CAL) 86-10 was issued December 30,1988 as a letter from William T. Russell:(NRC Region 1) tor.G. Bird (BEco). This supplement indicated that CAL conditions had been satisfied up to the point of carrying out plant restart and power ascension, and that successful implementation of

i the Power Ascension Test Program (PATP) was necessary for final closeout of the CAL. The supplement further indicated that following 4
completion of the PATP and licensee submittal of a report the staff wouldformitsindependentconclusionregardingprogrameffectiveness and would consider final closure of CAL 86-10. The licensee con.
cluded that the PATP was successfully completed and submitted their FinalAssessmentReport(FAR)ofthePATPonDecember 14, 1989.

3.2 Review of Final Assessment Report i
As part of its review of PATP completion and program effectiveness the NRC staff reviewed and evaluated the licensee's FAR. Thestaff's 4
review was coordinated through the NRC Pilgrim Restart Assessment i

The need for these confirmatory inspections af ter NRC receipt of the FAR was. minimal in view of the extraordinary inspection efforts that had been applied throughout the PATP.
The staff concluded that the FAR represents an acce) table self-assessment and indicates successful completion of tie PATP. The con-tent of the FAR is sumary rather than detailed. }iowever, it is well supported by backup documentation maintained by the licensee.
The licensee managers are knowledgeable n,J well-versed in the issues i
involved'-- they were able to provide detailed answers to staff cues-

tions during the management meeting and other staff questioning tur-ing the PATP. The licensee FAR provides a slightly more positive view of overall performance than NRC inspection documentst however, L
the difference is not significant. The. licensee's finel assessment agrees closely with the staff's assessment regarding safety signi-ficarit areas to be tar management attention. geted for-further improvement and continued The FAR also identifies needs for streamlining administrative processes for the sake of efficiency; licensee plans for approaching these areas include measures to preclude degradations in the level of safety assurance. This will be evaluated in the 1

3.3 Power Ascension __ Test Program Completion Previous NRC assessments have documented satisfactory licensee com-l 4
pletion of the PATP up to the 755 approval point.
Staff inspection and review activities have now concluded that-the licensee.has com-pleted the PATP through full power, including plant power ascension, testing, periods of normal operation ~and a management assessment.
Where the licensee has made changes to the details of the program, they have met the NRC staff stipulations in the CAL 86-10 Second Supplement, dated December 30, 1988.
NRC inspection activities re-lated to the final phase of the PATP, operation to full power, will i
be documented in NRC Inspection Reports 50-293/89-12 and 89-13.
t Based on these reviews and on staff review of the FAR described in I
paragraph 3.2 above the staff concludes'that the licensee has suc-l cessfully completed,the PATP.
3.4 Licensee $atisfaction of CAL 86-10 The CAi. 86-10 Second Supplement indicated that successful completion of the PATP was necessary for final closeout of CAL 86-10. As dis-cussed above, the. licensee has'successfully completed the PATP and submitted an acceptable report. IV. Conclusion l
The NRC Restart Assessment Panel met on January 4, 1990 to review the lic-ensee's completion of the Power Ascension Program and the Final Assessment Report. Boston Edison Company (BEco) had com)1eted the Power Ascension Program and submitted their Final Assessment leport.of the Power Ascension Program on December 14, 1989.
The NRC Staff reviewed the licensee's Final Assessment Report and concluded that the Final Assessment Report repre-sents an acceptable self-assessment and indicates successful completion of the Power Ascension Program.
In addition, Staff inspection and review activities have now concluded that the licensee has completed the Power Ascension Program. The Confirmatory Action Letter 86-10 Second Supplement indicated that successful completion of the Power Ascension Program was necessary for final close out of CAL 86-10. Based on the above, the Staff concludes that CAL 86-10 may be closed.
The NRC also concludes that with the completion of the Power Ascension Program,submittaloftheFInalAssessmentReportandcompletionofthe i
