ANNUAL ASSESSMENT LETTER FOR PILGRIM NUCLEAR POWER STATION (REPORT 05000293/2014001)
Dear Mr. Dent:
On February 9, 2015, the U. S. Nuclear Regulatory Commission (NRC) completed its end-of-cycle performance review of Pilgrim Nuclear Power Station (Pilgrim). The NRC reviewed the most recent quarterly performance indicators (PIs) in addition to inspection results and enforcement actions from January 1, 2014 through December 31, 2014. This letter informs you of the NRCs assessment of your facility during this period and its plans for future inspections at your facility.
On December 31, 2014, performance at Pilgrim was within the Degraded Cornerstone Column of the NRCs Reactor Oversight Process (ROP) Action Matrix due to two White PIs related to scrams in the Initiating Events cornerstone. The NRC determined the performance at Pilgrim during the most recent quarter remained in the Degraded Cornerstone Column of the NRCs ROP Action Matrix because two parallel White findings were opened in the Initiating Events cornerstone due to the objectives of Inspection Procedure 95002, Supplemental Inspection for One Degraded Cornerstone or Any Three White Inputs in a Strategic Performance Area, not being met for the two White PIs. The results of the 95002 inspection were issued on January 26, 2015 (Agencywide Documents Access and Management System (ADAMS)
Accession No. ML15026A069).
Since both of the White PIs have returned to Green in the first and third quarters of 2014 respectively, the parallel White findings will become effective during the quarter that the PIs returned to Green. These two White parallel inspection findings will not be removed from consideration in the NRCs assessment program until the identified weaknesses are addressed and corrected. The NRC will plan to resume the supplemental inspection, in accordance with Inspection Procedure 95002, after you have notified the NRC of your readiness. Specifically, this inspection will verify that Entergy has taken action to: (1) address the specific deficiencies in execution of corrective actions; (2) address the deficient cause evaluation; and (3) understand why corrective actions intended to address the identified corrective action program (CAP)
implementation weakness were not effective at ensuring the inspection objectives were satisfied. Additionally, the NRC will continue to assess the effectiveness and sustainability of your efforts to address challenges in CAP implementation during the next biennial problem identification and resolution inspection, scheduled for August 2015. Additionally on January 27, 2015, Pilgrim experienced a scram with complications due to a loss of both 345kV offsite electrical sources during a winter storm. On February 2, the NRC initiated a Special Inspection Team to inspect the performance of your operating crews and safety systems during the event. The results of this inspection have not yet been finalized but will be published in Inspection Report 05000293/2015007.
The enclosed inspection plan lists the inspections scheduled through June 30, 2016. Routine inspections performed by resident inspectors are not included in the inspection plan. The inspections listed during the last nine months of the inspection plan are tentative and may be revised at the mid-cycle performance review. The NRC provides the inspection plan to allow for the resolution of any scheduling conflicts and personnel availability issues. The NRC will contact you as soon as possible to discuss changes to the inspection plan should circumstances warrant any changes. This inspection plan does not include security-related inspections, which will be sent via separate, non-publicly available correspondence.
In accordance with Title 10 of the Code of Federal Regulations, Part 2.390 of the NRCs Rules of Practice, a copy of this letter will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records component of NRC's document system (ADAMS).
