PILGRIM STATUS REPORT FOR THE PERIOD MAY 20 - JUNE 2, 1989 Enclosed is the Pilgrim bi-weekly status report from the NRC Resident Of fice at Pilgrim.

The NRC Restart Staff is in place and was on-site monitoring licensee activities on an around-the-clock basis during plant startup and on an extended shift basis during other periods.

The Pilgrim bi weekly status report is intended to provide NRC management and the public with an overview of plant activities and NRC restart inspection activities.

Subsequent inspection reports will address rnany of these topics in more detail.

l, ENCLOSURE Pilgrim Status Report for Period May 20 - June 2,1989 1

1.0- Plant Status As of 8:00 a.m. on June 2,1989, the reactor was operating at approxi-mately 25% power with the turbine generator synchronized to the grid.

- 2.0 Power Ascension Test Program Status The licensee has completed the original scope of the Power Ascension Test Program up to the 25% power plateau.

Prior to continuation of the power ascension program to the 50*; power plateau, the licensee must obtain the NRC Region I Regional Administrator's concurrence.

The program includes NRC approval points at 5%, 25P4, 50% and 75?; of full power and both a licensee written report and NRC review after completion of testing at full power. The NRC Restart Assessment Panel will recommend to the Regional Administrator when the plant is considered ready to be released from each approval point.

3.0 Facilities Operations Summary As reported in the last Pilgrim Status Report (May 6-19,1989), the plant was in cold shutdown at the beginning of this report period while the licensee completed troubleshooting and other maintenance items following the turbine trip and resultant reactor scram on May 3, 1989.

At 5:25 a.m. on May 25, 1989, the licensee brought the reactor critical.

The turbine generator was synchronized to the electrical power distribu-tion grid at 8:35 a.m. on May 26, 1989.

On May 31, 1989, the licensee successfully completed the Reactor Core Isolation Cooling (RCIC) system testing as committed to by the licensee in their corrective actions for the RCIC Overpressurization event of April 12, 1989.

This test demonstrated the RCIC injection check valve forward flow capability and verification of closure following a vessel injection.

The plant remained at approximately 25% power throughout the remainder of this report period.

The licensee continued to conduct routine surveillance along with preventive and corrective maintenance.

4.0 ' Items of Special Interest Public Demonstration On May 29,1989, a group of about 30 people held a demonstration outside Pilgrim. The meeting was peaceful with no arrests.

NRC Restart Assessment Panel Teleconference A NRC Restart Assessment Panel teleconference was held on June 1, 1989, at the NPC Region I Office in King of Prussia, Pennsylvania. NRC management from Region I, NRR and the NRC resident staff participated.

The NRC Restart Panel meets periodically to coordinate planning and execution of NRC inspection and licensing activities related to Pilgrim. The Panel is chaired by Mr. Samuel J. Collins, Deputy Director, Division of Reactor Projects, NRC Region I.

5.0 Emergency Notification System (ENS) Reports Engineered Safety Feature (ESF) Actuations On May 20,1989, and again on May 21, 1989, while licen>ee personnel were performing a surveillance test on the recirculating motor generator set lockout relays, the licensee experienced an ESF actuation. The actuations occurred when a relay was inadvertently actuated during a surveillance test of the system logic, signaling one Residual Heat Removal (RHR) system valve to close and one RHR system valve to open.

Licensee personnel promptly restored normal equipment lineup following each actuation.

The licensee informed the NRC via ENS, Detailed NRC review of the above actuations and licensee actions will be documented in Inspection Report No. 50-293/89-06.

Reactor Water Cleanup System Isolation On May 25, 1989, an automatic isolation of the Reactor Water Cleanup (RWCU) system occurred. The isolation of the RWCU system resulted from actuation of a differential pressure instrument that initiates closure of the outboard isolation valves.

The instrument actuation was attributed to the collapsing of air bubbles in the instrument sensing line. Detailed NRC review of the isolation will be documented in Inspection Report No.

50-293/89-07.

Malfunction of Traversing In-Core Probe Limit Switch On May 31,1989, while performing a plot of neutron flux versus axial position in the. ore using the traversing in core probe (TIP) system, one of the TIP probes was withdrawn approximately two feet beyond the shield chamber due to a malfunctioning limit switch.

The shield chamber is used l

to store the TIP when not in use to provide shielding from the irradiated l.

detector. The. shield chamber is in the TIP room, at least 12 feet from a 30-inch concrete shield wall to ensure adequate radiation protection for personnel in the reactor building.

The primary containment isolation valve also remained open due to the mal-functioning limit switch.

Licensee personnel promptly isolated and elec-trically disarmed the primary containment isolation valve and re-inserted the traversing in-core probe back into the shield chamber. Reactor build-

. ing general area radiation levels adjacent to the TIP room were 3-5 mr/hr, therefore, entry into the licensee's Emergency Plan was not required. No personnel exposure occurred. Detailed NRC review of this event will be documented.in Inspection Report No. 50-293/89-07.

'6.0 Pilgrim Restart Staff Activities The Pilgrim restart staff was in extended coverage at the beginning of this report period, consistent with reduced testing activity. The restart staff began around-the-clock shif t coverage at 10:00 p.m. on May 23, 1989.

This coverage was reduced to extended shift coverage on May 26,1989 at 10:00 p.m.

Additional personnel from NRC Region I, the Office of Nuclear Reactor Regulation (NRR) and a NRC contractor were onsite during the report period to augment the resident inspection staff.

A NRC-developed inspection plan addressing the 5-25*; power plateau and the primary functional areas of operations, maintenance, surveillance, and startup testing continued to be implemented.

The inspectors assessed operator performance, organizational interf aces, adherence to procedures and technical specification compliance.

The Pilgrim Restart Staff continued to evaluate the performance of licen-see personnel and the plant to determine whether the licensee should be permitted to proceed to the next NRC approval point, a maximum of 50%

rated power. The Restart Staff and the Pilgrim Restart Assessment Panel will review the April 12, 1989 RCIC system event and May 3,1989 turbine trip / reactor trip event before finalizing that determination.

Contact with the press and public continued throughout the period. The NRC Restart Manager and the Region I Public Affairs Officer have been handling inquiries from the press and the public.
