THREE MILE ISLAND 1 PERIODIC CILRT SCHEDULE (AITS F14545Hl)

Inspection 50-289/77-11 Section III.A.6.(a) of Appendix J to 10 CFR 50 states that:
"If any periodic Type A test fails to meet thc applicable acceptance criteria in III.A.5.(b), the test schedule applicable to subsequent Type A tests will be reviewed and approved by the Commission".
Detail 8.a of the referenced report describes the initial failure of the first periodic Type A test (CILRT) at Three Mile Island-1 on April,16 and April, 17, 1977 The failure appears to be due to packing and fitting leaks corrected during the two day period.
Definitive analysis was precluded by repair of the leaks without prior local leak rate measurement and significant diurnal variations in the measured cor.tain-ment leak rate. To provide continued assurance of containment accept-ability between Type A tests, Region I recommends that either the inter-val between Type A tests be shortened or the Three Mile Island Type B and C testing program be upgraded to assure regular measurement and correction of possible leakage paths of the types identified during this and subsequent Type A tests. Also, this Type A test failure needs consideration when the exemptions from Appendix J, that this licensee has requested, are evaluated by the NRC.

Inspection 50-289/77-11 This refers to the inspection conducted by Mr. J. Higgins of this office on April 4-19, 1977, at Three Mile Island Nuclear Station, Middletown, Pennsylvania, of activities authorized by NRC License No. OPR-50 and to the discussions of our findings held by Mr. Higgins with Mr. J. Colitz of your staff at the conclusion of the inspection, and to a subsequent telephone discussion between Mr. Higgins and Mr. E. Lawrence of your staff on April 20 and 21,1977.
Areas examined during this inspection are described in the Office of Inspection and Enforcement Inspection Report which is enclosed with this letter. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, observations by the inspector, and independent calculations by the inspector.
Based on the results of this inspection, it appears that one of your activities was not conducted in full ccmpliance with NRC requirements, as set forth in the Notice of Violation, enclosed herewith as Apper. dix A.

(1) corrective steps which have been taken by you and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compli-ance will be achieved.

Any such application must be accompanied by an affidavit executed by the owne af the information, which identifies the document or part sought to be withheld, and which contains a statement of reasons which addresses with specificity the items which will be considered by the Commission as listed in subparagraoh (b)(4) of Section 2.790.
