Licensee Action on Previous Inscection Findinas (Closed) Infraction (77-18-01): Inadequate maintenance of emergenc The inspector held discussions with licensee personnel equipment.

and reviewed appropriate records to verify that the licensee had implemented a revised task assignment program, to include manual removal of completed tasks. The inspector noted that full imple-mentation of the revised task assignment program was completed by September 1,1977. The inspector stated that no further licensee action was required at this time.

(Closed) Infraction (77-18-02): Failure to perform dosimeter cali brations. The inspector noted that a revised procedure had been implemented by the September 1,1977 commitment date.

The inspec-tor informed the licensee that corrective actions were completed as outlined in a licensee letter to the NRC dated August 14, 1977.

(0 pen) Unresolved Item (76-27-01 and 76-18-01): Training program for offsite agencies.

The inspector discussed this item with licensee personnel and noted that the status of the licensee's action toward resolution was similar to that detailed in Office of Inspection and Enforcement Inspection Report Number 50-289/77-19 and 50-320/77-24, i.e., program not yet finalized.

The inspector reiterated that this item would remain unresolved pending sub-sequent review of the completed program by the NRC and that resolu tion was expected prior to the issuance of an cperating license fo Unit 2.

. (0 pen) Unresolved Item (77-24-01): Implementation of an Emergency Plan for Unit 2.

This item was reviewed in detail by the inspector with responsible licensee individuals. The inspector noted that, while actions were under way to implement a revised Emergency Plan ' covering Unit 2, these actions were still incomplete.

Details of Unit 2 Emergency Plan implementation are presented in Paragraph 3.

Unit 2 Emercency Plan Imolementation The inspector held discussions with responsible licensee individuals and performed a detailed review of the licensee's Emergency Plan Implementing Procedures formulated for the two unit Three Mile Island Site.

The inspector noted that various emissions and short-ccmings still remaired. With regard to the licensee's emergency facilities and equipment unique to Unit 2, the inspector noted that there were many either not yet in place or fully operational.

Licensee representatives stated that they were aware of the noted procedural and equipment shortccmings and that revisiens to pro-cedures and installation of additional equipment and supplies were underway. The inspector stated that, based upon the present status, additional review by the NRC would be required in all emergency planning areas prior to issuance of a Unit 2 operating ~ license.

[EiW The staff of the President's Commission on the Accident f?. at Three Mile Island is gathering facts directly related to e the event itself and also is collecting information of a back-Ms ground nature that rela'tes to the safe operation of reactors.

(.] ,One such set of background data is a listinc, discussion and analysis of reactor accidents < incidents and events of safety slanificance that have occurred in the US and abroad in the past 25 or 30 vears.

We cannot locate such a report and ask your assistance (and others) in gathering information that will allow it to be created.

A first step is simply to obtain a listing of accidents that are recollected by senior members of the NRC i staff or can be readily identified in existing documents.

.. Discussion and analyses can be prepared later.

At this stage we ask only for the minimum data-reactor, date, location, nature of the accident, comments on significance and/or con-sequences, and, if possible, references.

We expect that the memory of a number of persons will create a complete or near complete list when combined.

Criteria for inclusion in such a listing can be defined by measures relating to human injury, radiation release, dollar cost of repair, cost of replacement power (if an electric plant), down time of reactor, simply the magnitude of embar-rassment to the owner, or others.

We do not have such criteria quantified and ask your advice.
