An entrance interview was conducted at the site on January 27, 1977 with the Unit 1 Superintendent and Lead tiechanical Engineer.

During this meeting the inspector described the scope, expected duration, area and system to be inspected and records to be reviewed as part

of the inspection.

The inspector reviewed records, correspondence, held discussions with licensee personnel and inspected the work area after all repairs were completed to evaluate the adequacy of licensee actions in resolution of underground concrete piping problems

reported in LER No. 76-42/lP dated December 7,1976.

Various piping failures occurred on three underground pipes located between the intake pump / screen house north wall and the first

'

i elbow thrust blocks.

These pipes are identified as:

-- 20" RR - Reactor Building Emergency Coolir.g River Water

-- 24" DR - Decay Heat River Water

-- 30" NR - Nuclear Services River Water Records indicate that the 20" RR line bell fitting at the wall joint was cracked, the 21" underground piping cracked outside the screen house wall joint, and the 30" NR pipe spigot's clamp bar weld at the wall joint was also cracked.

Settlement had occurred at the thrust blocks of these lines.

These apparent movements and/or improper alignment during installation probably caused pipe stresses which resulted in the cracking.

The length of underground pipe run from the north wall of the screen house to the elbow thrust block is approximately 35',

12' and 8' respectively.

The 24" OR line is located between the other two lines--the adjacent 20" RR line on the west

side and the adjacent 30" NR on the east side.

problem Disposition - Repairs Undertaken The inspector's review of records and correspondence verified licensee actions in the problem analysis, disposition, and the corrective actions instituted to adequately restore the pipe i

lines to seismic class 1 status.

The 24" DR line was observed

'

leaking on October 22, 1976.

The entire line from wall joint to elbow was replaced by November 22, 1976. Further inspection of the adjacent 20" RR line revealed that the bell fitting on

,

i the wall joint was cracked and near failure. This line and the failed wall joint bell fitting were also replaced.

Further inspection also revealed the adjacent 30" NR line to be

,

i seeping at the pipe spigot's clamp bar weld. The latter was l

minor in nature and weld repair was accomplished satisfactorily.

Additional corrective action taken to restore the pipe lines j

to seismic class 1 status consists of the following.

measures were specified and the work was inspected by Gilbert

,.

Associates.

(1)

257 bags of portland cememt grout were injected beneath thrust blocks for the DR and NR lines through five vertical and inclined holes between them. (Screen house rests on

,

rock at + elev. 290, and surface grade is + elev. 305.)

(2) The NR thrust block was raised and a grout pad employing

.

160 - 55 lb. bags of Embeco #201 was installed beneath i

it to assure udequate support.

(3) The elbow joints nearest the wall on the three river water lines were freed from their encasement in the concrete of the thrust blocks to provide additional flexibility for these pipes to accept angular displacements.

(4)

Field testing of in-place soil underlyi1g the new 20" KR line demonstrated it has an ASTM relative density above 90%.

Also, the in-place density of the soil underlying the reinstalled 30" NR line was found to excced the minimum requirement of 70% relative density specified by Gilbert Associates.

Surveillance program to Monitor and Measure Possible Futurc, Settlement of the Three River Water Thrust Blocks Installation of " sounding rod" tubes on the three (3) thrust blocks, identified in licensee fourteen day report, provides for periodic surveillance of the elevation of these thrust blocks to detect any future settling.

The inspector reviewed survey level readings taken between January 7 and 21,1977.

The elevations were noted to be unchanged.

Continued readings

,

at two week periods will te taken to establish four data points.

If no change is observed, then readings at monthly intervals only

,

l will be taken. These will be forwarded to the inspector.

The inspector observed no discrepancies in these documents.

Licensee Actions to Establish Acceptability of Underground Pipes on South Side of River Water Intake Pump House i

The inspector reviewed rec 0rds, correspondence, and held discussions l

with licensee personnel to evaluate the adequacy of licensee actions to determine if underground pipe problems, similar to those on the i

(d north side of the river water pump house, could exist.

A supervised l

i j

test boring program was undertaken to establish the in-place density of the material beneath the thrust blocks of the Category 1 pipes

leading out of the south side of the Intake Structure.

The borings were advanced by the continuous standard penetration test (SPT)

method beneath four inch casing.

One boring was located between

'

the thrust blocks for the 24" DR and 30" NR pipes and a second boring was located between the 24" DR and 20" RR pipes.

These indicate the soil beneath the thrust blocks to be from dense to very dense.

Gilbert Associates interpreted the borings as indicating

'

the soil beneath the thrust blocks to be properly compacted prior

,

to the construction of the thrust blocks and that no undue settle-i l

ment should occur.

The inspector observed no discrepancies in these documents.

Westinghouse (W) and their corrective actions in response to the notice on bearing failures in certin W vertical motors used as RHR pump drivers.

W notifed the licensee in bulletin dated August 31, 1976.

Mel Ed acted on this notice and performed the necessary inspection to verify that proper locknuts and washers had been installed originally.
