Independent Inspection Effort Construction Status The 'A', 'B' and 'C' cubicle steam generator girth welds are complet The NDE indicated that they are satisfactor The reactor coolant piping to 'A', 'B' and 'C' steam generators are reinstalled and have passed the ND Preparations are being made for the hydro static test of the 'B' steam generato Engineering Task Assignment (ETA)

The inspector examined ETA 20003 "Transport of Steam Generator from the Dock to Station".

This ETA was reviewed for sign-offs, dates, incorporation of field changes, documentation and procedure completenes No item of noncompliance or deviations were identified in the area inspecte Observation of Safety-Related Work Activities Most of the time spent during this inspection involved the inspector's observation of various construction activities in Unit 2 containmen The following areas were observed:

Improper maintenance of cleanliness techniques were observed in eight of the areas inspecte A total of eight flanged nipples on reactor coolant pumps were not made up, left uncapped and unattende These flanged nipples tie the seal water coolant lines to the pump seal Work in progress in the containment could have allowed deposition of foreign materials in the clean componen CFR 50, Appendix B, Criterion II states in part,

"Activities affecting quality shall be accomplished under suitable control condition Controlled conditions include the use of appropriate equipment; sui'table environmental conditions for accomplishing the activity, such as adequate cleanliness; and assurance that all prerequisites for the given activity have been satisfied." VEPCO topical report VEP-1-A, Amendment 3 paragraph 17.2.0.2 states that VEPCO Operational Quality Assurance Program shall conform to ANSI N45. Paragraph 5 of ANSI 45.2.2 states in part, "Openings and pipe ends shall be sealed at all times except when they must be unsealed to carry out necessary operations".

Improper rigging techniques were observed in two of the areas inspecte In one area, the construction craft was rigging a large section of shroud coolant piping from the 'C' steam gener-ator feedwater pipin In the other area, the construction craft was rigging ventilation ductwork from the seismic supports to the

'C' steam generator reactor coolant pum CFR 50, Appendix B, Criterion XIII states in part, "... measures shall be established to control the handling, storage, shipping, cleaning and preser-vation of material and equipment in accordance with work and inspection instructions to prevent damage and deterioration.

Daniel Construction procedure WP-108, Revision 1, paragraph states in part, "... rigging practices shall follow generally accepted standards to insure the safety of personnel and the proper usage of equipmen The above two conditions are identi-fied as infraction 281/79-81-01, "Improper rigging techniques".
