CPPR-93 10CFR50.55(e) CONDITION #269 BECHTEL REPAIR OF WRONG WELD AREA INTERIM REPORT Description of Deficiency Field Weld RHR851-18.19 W12AR1 was originally rejected by RT examination for unacceptable weld defects. Due to an error in laying out the weld area to be repaired, the wrong weld area of the weld was repair-welded and radiograph-ed. The repair radiographs were not compared against the original radiographs and, therefore, were accepted without noting that the defects had not been corrected.

Safety Implication Rather than perform a detailed safety analysis to prove that *he defects would not causa a failure in a safety related system, the Project J chosen to invoke subsection iv of 10CFR50.55(e). Based on this decision, we assume that the defects identified in weld RHR851-18.19 W12AR1 could have caused a failure in the safety related RHR system. Therefore, this condition is considered reportable under the provisions of 10CFR50.55(e).

Cause QA Program breakdown on the part of the Construction Manager (CM), Bechtel Power Corporation, in that inadequate measures were implemented to ensure defects outside Code allowables identified during radiography were removed.

Corrective Action Nonconformance Report (NCR) 021688 was issued to document the above identi-fied deficiency on weld RHR 851-18.19 W12ARI. Weld RHR 851-18.19 W12AR1 has been reworked to remove the originally identified defects, reradiographed and accepted.

i i Action to Prevent Recurrence For all future welds, the Construction Manager will compare the radiograph from the repair area with the original radiograph to ensure that the proper r weld area was repaired and the identified defects removed.

l To ensure that other welds with the wrong area repair did not exist, the Con-struction Manager has reviewed all welds repaired under the Bechtel program.

During this review, it was discovered that one (1) additional weld (RHR1968-5 FW17RI) had the wrong weld area repaired, and the original defects still existed. NCR 21767 has been issued to document this nonconformance.

A total of 326 weld repairs were reviewed. During the review additional prob-lems were identified, as follows,

a. 2 welds were rejected for wrong weld area repaired (the original weld discovered, plus weld RHR 1968-5 FWl7RI).

b. 4 welds.in which the original defect was not removed by the the repair.
c. 3 welds in which the weld repair radiograph could not be located.
d. 10 welds in which rejectable indications existed in the repair area.

Corrective Actions

a. NCR's 21688 and 21767 have been issued on welds RHR851-18.19 W12AR1 and RHR1968-5 FW17R1, respectively.
b. NCR's have been issued to document the nonconforming conditions.
c. The radiographs were located or the area reradiographed.
d. NCR's have been issued to document the nonconforming conditions.

All NCR's will be resolved in accordance with site approved procedures.

Items b and d warrant further investigation by the Project as to the root cause and will be discussed in our final report.
