Missed Surveillance - No. 23 and No. 24 Accumulator Levels.

This report was jnitiated by Incident Report 82-472.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 3% - Unit Load 0 MWe.

DECCRIPTION OF OCCURRENCE:

At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, November 19, 1982, the shift supervisor realized that the surveillance requirement to check the local sightglass levels of Nos. 23 and 24 Accumulators was overdue. The previous check had been performed at 1705 hours0.0197 days <br />0.474 hours <br />0.00282 weeks <br />6.487525e-4 months <br />, November 18, with the next one due at 0505 hours0.00584 days <br />0.14 hours <br />8.349868e-4 weeks <br />1.921525e-4 months <br />, November 19. Adding the 25% allowable extension, it was ultimately due at 0805 hours0.00932 days <br />0.224 hours <br />0.00133 weeks <br />3.063025e-4 months <br />. Upon discovery of the oversight, an operator was sent into containment to obtain the levels. At 1441 hours0.0167 days <br />0.4 hours <br />0.00238 weeks <br />5.483005e-4 months <br />, the levels on Nos. 23 and 24 Accumu-lators were verified as satisfactory. Non-compliance with Surveil- -

lance Requirement 4.5.1.a (1) exceeded the limits of LCO 3.5.1.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the missed surveillance was personnel oversight.

ANALYSIS OF OCCURRENCE:

Surveillance Requirement 4.5.1.a(1) requires:

Each accumulator shall be demonstrated operable at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> by verifying the water level and nitrogen cover-pressure in the tanks.

LCO 3.5.1.b requires:

Each reactor coolant system accumulator shall be operable with a contained volume of between 6223 and 6500 gallons of borated water.

ANALYSIS OF OCCURRENCE: (continued)

The operability of each reactor coolant system (RCS) accumulator ensures that a sufficient volume of borated water will be immediately forced into the reactor core through each of the l

cold legs in the event the RCS pressure falls below the pressure of the accumulators. This initial surge of water into the core provides the initial cooling mechanism during large RCS pipe ruptures.

j CORRECTIVE ACTION: -

i The levels on Nos. 23 and 24 Accumulators were verified as satis-factory. The shif t personnel involved were counseled as to the importance of prompt completion of required surveillance procedures.

Design Change Request 2EC-1424, to replace the level transmitters and install additional valves, has been submitted for instellation during the next refueling outage.
