Containment Cooling System -

No. 22 Containment Fan Coil Unit -

Inoperable.

This report was initiated by Incident Report 82-230.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 82% -

Unit Load 890 MWe.

DESCRIPTION OF OCCURRENCE:

On August 12, 1982, during routine operation, the Control Room Operator received indication of a possible service water leak on No. 22 Containment Fan Coil Unit (CFCU).

A containment entry was made and a sample of condensate from the CFCU was taken.

The condensate flow rate was approximated to be.25 gallons per hour, however, no leak of the service water piping could be located.

No. 22 CFCU was isolated and declared inoperable at 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />.

The NRC was* notified by telephone at 1047 hours0.0121 days <br />0.291 hours <br />0.00173 weeks <br />3.983835e-4 months <br />, in accordance with NRC Bulletin 80-24.

Because No. 25 CFCU was already inoperable, Limiting Condition for Operation Action Statement 3.6.2.3b was entered at 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />, August 12, 1982.

Both containment spray systems were operable throughout the occurrence.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Maintenance investigated the problem and found no leak on the service water piping.

A chloride analysis of the condensate from No. 25 CFCU indicated only 20% of the chlorides found in normal service water.

Therefore, it was determined that there was no service water leak on No. 25 CFCU, and that the indidations of this occurrence represented condensation on the CFCU cooling coils.

The presence of chlorides in the sample was attributed to a trace amount of chlorides on the CFCU cooling coils from previous service water leaks.

ANALYSIS OF OCCURRENCE:

The CFCU's operate in conjunction with the containment spray systems to remove heat and radioactive contamination from the containment atmosphere in the event of a design basis accident.

Operability of either all fan coil groups or of both containment spray systems is necessary to ensure offsite radiation dose is maintained within the limits of 10CFRlOO.

ANALYSIS OF OCCURRENCE: Although two groups of CFCU's were inoperable, redundant containment cooling capability was provided by the containment spray systems.

Therefore, this occurrence involved no risk to the health and safety of the general public.

Due to inoperability of the CFCU's, this event constituted operation in a degraded mode permitted by a limiting condition for operation, and is reportable in accordance with Technical Specification 6~9.1.9.b.

Action Statement 3.6.2.3b requires:

With two groups of containment cooling fans inoperable, restore at least one group to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />, and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

As noted, No. 22 CFCU was declared inoperable, isolated, and Action Statement 3.6.2.3b was entered at 1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />.

An investi-gation was conducted and it was determined that there was no service water leak on No. 22 CFCU.

Therefore, No. 22 CFCU was-declared operable and Action Statement 3.6.2.3b was terminated at 1455 hours0.0168 days <br />0.404 hours <br />0.00241 weeks <br />5.536275e-4 months <br />, August 12, 1982.
