  • no nn .j DAVIS SESSE ADMINISTRATIVE PROCEDURE PAGE REVISION PROCEDURE NUMBER Unescorted Access Requirements 4 03 IS-AC-00516 1.0 PURPOSE 1.1 To outline the security requirements which shall be met prior to granting an individual unescorted access to the Protected and/or Vital Areas of the Davis-Besse Nuclear 3 Power Station (DBNPS) or to individuals assigned to  ;

Emergency Response Organization (ERO) duties.

1.2 To ensure that individuals have been investigated to verify reliability and trustworthiness.

I 3.0 . APPLICABILITY / SCOPE 2.1 Aeolicabil'ity l This procedure applies to all individuals requesting unescorted access to the Protected / Vital Areas (either initially or renewing), individuals assigned ERO duties, and to those individuals involved in the processing.

2.2 Excentions 2.2.1 This procedure does not apply to NRC personnel requesting unescorted access. NRC personnel are processed in accordance with IS-AC-00011.

4.1 BACKGROUND

DOCUMENTATION - Information supplied by the group, or agency, which performs any part(s) of the screening requirements specified in subsection 6.2, Unescorted Access Requirements and in accordance with the l Fair Credit Reporting Act and other applicable laws.

4.2 BACKGROUND

VERIFICATION - Verification may be performed through correspondence, telephone contact or personal contact. As a minimum, the name, title (if applicable) address, telephone number, date of contact and the data obtained shall be included. Information requested which former employers / organizations will not supply, shall be documented to indicate that the information was requested but was not received.

4.3 COMPANY PERSONNEL - Individual employed by Centerior Energy Corporation or its subsidiaries.

4.4 CONTINUOUSLY EMPLOYED - Uninterrupted employment l

l 4.5 CONTRACTOR CONTACT - An employee from the contracting company who is the designated contact for their company's I screening program.

1 4.6 CONTRACT PERSONNEL - Any individual not employed by Centerior Energy Corporation or its subsidiaries.

and/or vital Area access or placement on the ERO When possibly disqualifying information is supplied.

4.9 SUITABLE INQUIRY - The best effort verification of employment history for the past 5 years, but in no case less than 3 years, obtained through contacts with previous employers to determine if a person was, in the past, tested positive for illegal drugs, subjected to a plan for treating substance abuse, removed from, or made ineligible for activities identified within the scope of this procedure, or denied unescorted access at any other nuclear power plant or other employment in accordance with a fitness for duty policy.

4.10 UNESCORTED ACCESS - Tor the purpose of this procedure, unescorted access is defined to be unescorted access to the Protected and/or Vital Areas.

Unescorted Access Requirements 8 03 IS-AC-00516 6.0 PROCEDURE 6.1 General Reauirements 0.1.1 Prior to recommending an individual for unescorted Protected and/or Vital Area access, Centerior Corporate Security, shall ensure that background investigation results have successfully met the requirements specified in this procedure.

6.1.2 Individualo may withdraw consent for background investigation at any time during the screening l process.

6.1.3 Such withdrawal shall be in writing to the Supervisor-Access Control and considered a withdrawal of the request to be granted unescorted access.

6.1.4 The investigation process shall be stopped as soon as practical following withdrawal of an individual's request.

6.1.5 Each individual investigated by Centerior Corporate l Security shall sign the applicable release of I information form prior to the start of the investigation.

6.1.6 Prior to granting unescorted accer.s to a Company Employee, the Supervisor-Access Control, or designee, shall verify that Attachment 1, Company Request for Unescorted Access, is completed.

NOTE 6.1.7 Contract personnel are subject to the same back-ground investigation requirements. Tne prime supplier /cantractor or employer is responsible for compliance with this procedure and documentation.

6.1.7 Employers of contract personnel shall ensure:

a. Their personnel requesting unescorted access are investigated to the requirements of this l procedure.

b. That evaluations of background information are

-performed in accordance with subsection 6.5.

c. Documentation verifying' successful completion of the requirements of this procedure is received and on file prior to submittal.of Attachment 2,l Contractor Request for Unescorted Access.
d. Attachment 2 is submitted for each employee requesting unescorted access.

6.2 Unescorted _ Access _Reauirements 6.2.1 Company and contract personnel shall complete the following to obtain initial unescorted access at DBNPS:

-a. Background investigation as described in steps  !

6.2.3 and 6.2.4.

b. Drug and alcohol testing as d3JCribed in step 6.2.5.
c. Psychological evaluation as described in step 6.2.6 and 6.2.7. l
d. FBI criminal history check as described in step 6.2.8.
e. Complete Fitness For Duty _ Questionnaire.

a. Education History Verification shall be performed by:
1. Verifying the individual's claimed enrollment at an educational institution for the previous 5 years.
2. Verify the highest claimed level, post high school, attendance or degree, regsrdless of time.
b. Verification of true identity by one of the following means:
1. Photo identification
2. Certified copy of birth certificate
3. A comparison of physical characteristics with employment, education and military and other records.
4. Employers or character references who have a personal acquaintance with the applicant.
c. Military service, within the previous 5 years, shall be verified through the National Porsonnel Records Center (NPRC) by receipt of either: C-2
1. Copy of Form DD214 C-2]
2. Copy of other military records C-2l e If it becomes known that the individual's I discharge was other than honorable, based solely on receipt of the form DD214, further investigation shall be made, e Interim (180 days or less) unescorted access may be granted to individuals prior to receipt of a Form DD214 or other military records from the NPRC if all other applicable background screening elements are met and the request for military records is C-2 documented w -

d. Employment history verification shall be obtained by contacting the previous employers for the past 5 years to obtain the following informationt l ROTE _6.2.4.d.1 Because of the multiple employments experienced -

during a 5 year period, especially by contractor personnel, verification of all such employments may '

not be possible. Every reasonable-attempt shall be made, and documented to cover the entire 5 year >

period. However, applicants for unescorted access may be considered based on a 3 year employment ,

2. Disciplinary history.
3. Reasons for termination.and eligibility for rehire.

Suitable Inquiry for each employer which shall consist'of the following questions: l e-Tested positive for drug / alcohol use e Subject to an assistance plan for treatment of substance abuse (Except self-referral) e Removed from activities or denied access at another nuclear facility in accordance with a fitness for duty policy.

Character and reputation of the applicant shall be investigated through contact with two l references supplied by the applicant and at least two additional references, not related to the applicant, developed during the investigation. The references shall have known the individual for at least one year and been in contact with the individual at least once a month.- Emphasis shall be placed on:
1. Identified psychological problems. ,
2. Criminal history.
3. Illegal use or possession of controlled i substances. 2
4. Abuse of alcohol. '
5. susceptibility to coercion.
6. Any other conduct relating-to an applicant's trustworthiness or reliability to discharge job dutias within the environment of a nuclea; power plant,
f. Credit checks shall check the applicant's credit history through contacts with credit bureaus

b. The illegal use or possession of a controlled substance, or alcohol abuse, without adequate evidence of rehabilitation (i.e. confirmation of completion of a rehabilitation program).
c. Evaluation of the FBI criminal history check shall be in accordance with IS-AC-00015, Fingerprint Processing and Controls.

NOTE _6.5.1.d Medical certification of recovery does not automatically guarantee eligibility for unescorted access.

d. Any history of mental illness or instability without medical certification of recovery which is attested to by a clinical or licensed psychologist.
e. Any evidence that the individual is susceptible to outside influences to commit sabotage or other actions that would adversely reflect on the individual's reliability,
f. Any evidence that the individual has committed, has attempted to commit, or has aided someone to commit, any act of sabotage or other actions which would adversely reflect on the individual's security reliability and trustworthiness.
g. A documented psychological evaluation provided by a clinical or licensed psychologist that indicated that the individual is not security reliable in a nuclear power plant environment.

h. If the individual in the past tested positive for illegal drugs, was treated for substance abuse, or denied unescorted access at any other nuclear power plant or other employment in accordance with a fitness for duty policy.
1. Any information which would adversely reflect on the individual's security reliability and trustworthiness.

6.5.2 For individuals not investigated by Centeri]r Corporate Security, possibly disqualifying information shall be provided to the TE Supervisor-Access Control or designee.

6.5.3 The TE Supervisor-Access Control shall notify the Manager -Industrial Security of any possible disqualifying information.

6.5.4 For individuals investigated by Centerior Corporate Security, the Supervisor-Access Control, or designee, shall notify the Manager - Industrial Security of any possible disqualifying information.l 6.3.5 Upon notification of possible disqualifying l information, the Manager-Industrial Security shall place the case on the agenda of the next regularly scheduled meeting of the Industrial Security Committee.

6.5.6 The Industrial Security Committeo shall review the facts of the case and determine if the individual is security reliable.

6.5.7 If the Industrial Security Committee determines that the individual is security reliable, the Supervisor-Access control, or designeo, shall grant unescorted aCCOss.

6.5.8 If the Industrial Security Committee datermines that the individual is not security reliable, the Supervisor-Access Control, or designee, shall place the individual on the Denied Access List in accordance with IS-AC-00017, Danied Access List Control.

6.5.9 Individuals denied access may appeal their access status in accordance with IS-AC-00017.

a. Drug and alcohol testing within 60 days of granting the request.

d. Verification that the individual is at least 18 years of age.

e. A credit history check.
f. Recommendation of one developed reference who has had frequent and direct association with the applicant. (reference 6.2.4.e)
g. Submittal of fingerprints to the FBI as per IS-AC-00015.

-h. Documentation that the request for Form DD214 or other military records was submitted to the National Personnel Records Center (NPRC) if military service falls.within the previous 5 years. C-2 6.6.3 The complete background investigation, as described in section 6.2, shall be completed within the 180 days, even if there is no longer a need for unescorted access.

6.6.4 Interim unescorted access shall not be renewed, extended, or transferred to another licensee.

6.6.5 Personnel granted interim unescorted access shall submit a new Company Request for Unescorted Access or Contractor Request for Urescorted Access, to maintain access when the background investigation, as described in section 6.2, is completed.

6.6.7 If revocation of unescorted access is necessitated, a final determination shall be made in accordance with IS-AC-00017.

64 6.8 The Supervisor - Access Control, or designee shall

, notify the_ applicable Company Supervisor or contractor representative of the final determination.

6.7 Termination _of_ Unescorted _ Access 6.7.1 Contractor unescorted access shall expire at the end of the month one year after being granted unescorted access unless otherwise determined by the-Contractor Request for Unescorted Access Form.

6.7.2 The Toledo Edison Supervisor or Area. Head shall ensure Attachment 6, Badged Individual' Check-Out Form, is completed for all Company Employees or

-Contractor pcrsonnel who no longer require unescorted access to the Protected and/or Vital areas.

6.7.3 .W hen un individual no longer requires unescorted access or is ineligible for unescorted access due to cause,.the Supervisor or Contractor Contact shall l immediately notify the Supervisor-Access Control, or designee.

.6.7.4 -Written' documentation shall follow the verbal notification.

b. Receipt of Attachment 7, for 180 days, app';oved by the Plant Manager and Copervisor-Accesr, C-1 Control, or designee,
c. Receipt of Attachment 1, completed by the individual's Company Supervisor,
d. Receipt of Attachment 5, with applicable sections completed.
e. The complete background investigation, as described in section 6.2, shall be completed within the 180 dcys, even if there is no longer a need for unescorted accesr..

6.S.2 For individuals who currently possess or have C-1 possessed unescorted access at Davis-Besse within the previous 365 days and are transferring to employment other than Toledo Edison or Centerior, the Supervisor-Access control, or designee, may grant unescorted access upon:

a. Verification that previous access was favorably terminated, if applicable. C-1
b. Receipt of Attachment 7, for 180 days, approved by the Plant Manager and Supervisor-Access control, or designee.
c. Receipt of Attachment 2 from the individual's employer,
d. Receipt of Attachment 5 with applicable sections completed.

6.8.3 Transfer of unescorted access from Perry Nuclear Power Plant, Reciprocated Access, to DBNPS and DBNPS to PNPP.

a. Company personnel who currently possess unescorted access at Perry Nuclear Power Plant (PNPP) may request reciprocated unescorted access be granted at DDNPS.
1. Supervisor-Access Control at DBNPS, or designee, shall grant unescorted access to the individual requesting unescorted access:

e Upon receipt of Attachment 8, Qualification Verification Worksheet, from PNPP.

e completion of site specific training, o Completion of a Attachment 5.

NOTE: 6.8.3.a.2 Whole Body Count and NRC Form 4 may be required if not completed at PNPP or not verified on Qualification Verification Worksheet.

2. The individual shall submit Attachments 5 and 8 to the Access Control Clerk for processing,
b. Company employees who currently possess l unescorted access at DBNPS may request reciprocated unescorted access at PNPP.
1. Attachment 9, Request for Peciprocated Unescorted Accecs shall be completed as stated below:

2. Upon receipt of Attachment 9, the Supervisor-Access Control, or designee, shall:

e complete Section 3.

e Place the form into the PNPP' Information Booklet, e Provide the PNPP Information Booklet to the individual.

6.9 Transfer _of iUnescorted _ Access from_Another Licensee 6.9.1 .The Supervisor-Access Control may accept an individual's unescorted access authorization from another licensee and grant unescorted access for 180

-days provided the following information is verified:

a. -The individual currently holds, or has held, unescorted access within the previous 365 days

f. The individual's identity is verified by checking information such as name, birth date, social security number, gender, and physical characteristics,

NOTE:_6.9.2 This does not apply to individuals that are transferring via the INDEX system. Only verification of INDEX eligibility is required.

6.9.2 To maintain unoscorted access beyond 180 days, the individual's new employer shall complete the background investigation, as described in section 6.2, within the 180 days, even if there is no longer a need for unescorted access.

,9.3 After nteps 6.9.1.a through g have been completed Supervisor-Access Control or designee shall grant unescorted access upon:

a. Receipt of Attachment 2, completed by the individual's Supervisor,
b. Receipt of Attachment 5 with applicable sections completed.

6.9.4 Individuals whose previous unescorted access has been expired for 366 days or greater shall complete the screening requirements of suusection 6.2.
