Abnormal Occurrence Report No. 50-219/75/ 3 1

IDENTIFICATION Failure of containment spray pump 51A to start when called upon OF OCCURRENCE:

to do so. i

'Ihis event is considered to be an abnormal occurrence as de-fined in the Technical Specifications, paragraph 1.15D.

Steady State Power Routine Shutdown CONDITIONS PRIOR Hot Standby Operation

'IO OCCURRENCE: Load Changes During Cold Shutdown Refueling Shutdown Routine Power Operation x Routine Startup Other (Specify)

Operation Power: Reactor,1280 FMt Electric, 435 FMe 4

Flow: Recirc., S.1 X 10 gpm Feed., 4.64 X 10 6 lb/hr Stack Gas: 13,500 pCi/sec DESCRIPTION It was observed on Tuesday, February 11, 1975, at 0335, during I

i OF OCCURRENCE:

the monthly surveillance test of the containment spray system that containment spray pump 51A failed to start in the automatic mode when subjected to simulated low-low reactor water level i and high dr>vell pressure signals. It was observed that the

" pump failure" alarm initiated approximately 57 seconds after tha start signal was applied. In an attenpt to troubleshoot the cause of the failure, a recheck of the jumpering required to establish the starting logic was conducted with no abnormalties.

,(

Abn:rmal Occurrenes Pcga 2 I R2 port No. 5,0-219/75/3 Q

r discovered. The surveillance test was again conducted and the j i l) c pump was observed to start in the proper manner.

It should be noted that the failure monitor installed as the 8 result of an identical occurrence on October o21, 1974 was not  :

R connected during this event. In an e'ffort to determine the cause of the failure, the start sequence of the 51A pump was repeated four times with the failure monitor in service. In each of these operations, the pump started in the proper manner.

Design Procedure APPARENT CAUSE OF OCCURRENCE: Manufacture i Unusual Service Condition Installation / I Inc. Environmental Construction Compenent Failure Operator Other (Specify) n e cause of the occurrence is presently under investigation.

ANALYSIS OF De only significance associated with the failure of SIA to OCCURRENCE:

start is in a loss of system redundancy. Had conditions arisen requiring containment spray, Pump 51C would have started automatically and performed the containment heat removal func-tion. In addition, the manually initiated backup Pumps 51B for System 1 and 51D for System 2 were operable and could have' been initiated. The containment heat removal function can be ade-quately supplied by a single containment spray pump.

CORRECTIVE Since the failure of the pump to start did not repeat on the ACTION:

second surveillance test or the four subsequent start sequences, the cause of the initial failure was indeterminable.
